평가배경
Mineral Trioxide Aggregate (MTA)를 이용한 부분치수절단술은 부분적인 치수감염 또는 치수질환 환자를 대상으로 치수복조와 같은 천공부위의 수복에 사용하는 MTA를 이용하여 치수생활력을 유지하고, 치근 성장을 유도하기 위한 기술이다.
본 의료기술은 2013년 제8차 신의료기술평가위원회(2013.8.30.)에서 안전하고 유효한 기술로 인정되었으며, 이후 2021년 의료기술재평가 주제 수요조사를 통해 제안되었다. 2021년 제3차 의료기술재평가위원회(2021.3.12.)에서는 본 기술을 우선순위 심의평가를 통해 의료기술재평가 대상으로 선정하여 2021년 제4차 의료기술재평가위원회(2021.4.9.)에서 평가계획서 및 소위원회구성(안)을 심의하였으며, 2021년 제11차 의료기술재평가위원회(2021.11.12.)에서 최종심의를 수행하였다.
본 평가의 목적은 MTA를 이용한 부분치수절단술의 임상적 안전성 및 유효성 등에 대한 과학적 근거를 제공하고 이를 통해 의료기술의 적정사용 등 정책적 의사결정을 지원하는 것이다.
평가방법
MTA를 이용한 부분치수절단술에 대한 안전성 및 유효성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “MTA를 이용한 부분치수절단술 평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회는 구강악안면외과 1인, 치과보존과 2인, 소아치과 1인, 근거기반의학 1인의 전문가 총 5인으로 구성하였다.
평가의 핵심질문은 ‘부분적인 치수감염 또는 치수질환 환자에서 MTA를 이용한 부분치수절단술의 임상적 안전성과 유효성은 어떠한가?’ 이며, 안전성은 시술관련 이상반응 또는 부작용, 유효성은 시술성공률(치료 후 통증이 없거나 압통이 없는 경우 등 임상적 시술성공과 근관내 병소나 내흡수, 치근흡수가 없는 경우 등의 방사선학적 시술성공을 포함), 시술 실패율(치료 후 통증, 압통이 있거나 부종, 누공과 같은 증상을 보이는 임상적 시술 실패와 방사선학적으로 근간 내 또는 근간 외 병소가 있는 경우 등의 방사선학적 시술 실패 포함) 및 발치율로 평가하였다.
핵심질문을 토대로 문헌검색은 국외 3개, 국내 5개 데이터베이스에서 수행하였고, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 문헌을 선별하여 선택하였다. 문헌의 비뚤림위험 평가는 무작위 배정 임상연구의 경우 Cochrane의 Risk of Bias를, 비무작위 배정 임상연구의 경우 Risk of Bias for Nonrandomized Studies (RoBANS)를 이용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 소위원회를 통해 확인된 자료추출 서식을 활용하여 수행하였다. 자료분석은 질적 검토(qualitative review) 방법을 적용하였다. 체계적 문헌고찰 결과의 근거 수준은 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 접근 방법으로 평가하고, 평가결과를 토대로 권고등급을 결정하였다. 결론 도출은 MTA를 이용한 부분치수절단술에 대해 임상적 안전성 및 유효성에 대한 분석결과를 우선적으로 제시하고, 합의를 통하여 결론을 결정하였다.
평가결과
MTA를 이용한 부분치수절단술(이하 ‘중재군’)의 최종 선택문헌은 5편(무작위 배정 임상연구 4편, 비무작위 배정 임상연구 1편)으로 모두 수산화칼슘을 이용한 부분치수절단술(이하 ‘비교군’)과 비교한 연구였다. 비뚤림위험 평가결과 무작위 배정 임상연구에서는 25%에서 불완전한 결과자료 부분에 대한 비뚤림위험이 ‘높음’으로 평가되었으며, 비무작위 배정 임상연구 1편은 대상군 비교가능성, 교란변수, 평가자의 눈가림, 선택적 결과보고에서 비뚤림위험이 ‘불확실’로 평가되었다.
안전성
안전성 평가결과 1편에서의 치수괴사 및 감염률은 중재군 14.3% (1/7명), 비교군 5.4% (5/93명)으로 군간 통계적으로 유의한 차이를 보이지 않았다.
이에 소위원회에서는 MTA를 이용한 부분치수절단술의 안전성은 수산화칼슘을 이용한 부분치수절단술과 유사한 수준으로 판단하였다.
유효성
시술 성공률은 2편에서 중재군은 각각 90%, 93%, 비교군은 각각 93%, 91%로 군간 유의한 차이를 보이지 않았으며, 1편에서 중재군 85%, 비교군 43%로 중재군의 시술 성공률이 유의하게 더 높았다.
시술 실패율은 1편에서 중재군, 비교군 모두 11.8%로 군간 유의한 차이를 보이지 않았으며, 1편에서 중재군 15.3%, 비교군 56.5%으로 중재군이 유의하게 낮았다.
발치율은 1편에서 중재군 4.55%, 비교군 2.5%로 군간 유의한 차이를 보이지 않았다.
이에 소위원회에서는 MTA를 이용한 부분치수절단술은 시술성공률, 시술실패율, 발치율에 있어 수산화칼슘을 이용한 부분치수절단술과 유사한 수준의 유효한 의료기술로 판단하였다.
다만, 현재 평가에 선택된 비교연구 결과가 많지 않아 지속적인 연구가 필요하다는 의견이었다.
결론 및 제언
MTA를 이용한 부분치수절단술 재평가 소위원회는 현재 평가결과에 근거하여 다음과 같이 제언하였다.
MTA를 이용한 부분치수절단술은 수산화칼슘을 이용한 부분치수절단술과 유사한 수준의 안전하고 유효한 의료기술로 판단되나, 현재 평가에 선택된 비교연구결과가 많지 않아 지속적인 연구가 필요하다는 의견이었다.
2021년 제11차 의료기술재평가위원회(2021.11.12.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “MTA를 이용한 부분치수절단술”에 대해 다음과 같이 심의하였다.
MTA를 이용한 부분치수절단술은 수산화칼슘을 이용한 부분치수절단술과 유사한 수준의 안전하고 유효한 의료기술로 판단되나, 평가에 선택된 비교연구결과가 많지 않아 그 근거가 아직 제한적이라는 소위원회 의견에 동의하였다.
이에 따라 의료기술재평가위원회는 부분적인 치수감염 또는 치수질환 환자를 대상으로 MTA를 이용한 부분치수절단술이 수산화칼슘을 이용한 부분치수절단술과 유사한 수준의 안전하고 유효한 의료기술로 판단되나 그 근거가 제한적이어서 조건부 권고로 심의하였다.

Partial pulpotomy using mineral trioxide aggregate
Background
Partial pulpotomy is performed using mineral trioxide aggregate (MTA), a material used to manage perforation, such as for pulp capping, maintaining pulp vitality, and promoting dental root growth in patients with partial pulp infection or pulp disease.
This technique was recognized as safe and effective by the 8th Health Technology Assessment Committe in 2013 (August 30, 2013), but a reassessment was proposed based on demand survey for medical technology to be evaluated in 2021. The technique was selected for reassessment based on determination of the priority of the technology to be evaluated in the 3rd Health Technology Reassessment Committe in 2021 (March 12, 2021). The protocol was approved by the 4th Health Technology Reassessment Committee of 2021 (April 9, 2021) and a relevant subcommittee was designated and the 11th Health Technology Reassessment Committee of 2021(November 12, 2021) concluded the safety and effectiveness of the technique.
This assessment aimed to provide evidence on the clinical safety and effectiveness of MTA in partial pulpotomy and support policy decision-making related to the appropriate use of this technique.
Methods
A systematic literature review was performed to assess the safety and effectiveness of MTA partial pulpotomy. All assessment methods were finalized after a review by a subcommittee for the assessment of MTA partial pulpotomy (hereinafter “subcommittee”) based on the purpose of the assessment. The subcommittee comprised five experts: two in endodontics and one each in oral and maxillofacial surgery, pediatric dentistry, and evidence-based medicine.
The key question was “How is the clinical safety and effectiveness of MTA partial pulpotomy in patients with partial pulp infection or pulp disease?” Safety was assessed based on procedure-related side effect or adverse events, while effectiveness was assessed based on success rate (which included clinical success, defined as the absence of pain after the treatment, and radiological success, defined as the absence of root canal lesions and root resorption), failure rate (which included clinical failure, defined as the presence of pain after treatment or symptoms of swelling and fistula, and radiological failure, defined as the presence of root canal lesions or root resorption), and tooth extraction. Three international and five domestic databases were searched based on the key question. Two reviewers independently selected the articles in accordance with the inclusion and exclusion criteria. Risk of bias (RoB) was assessed independently by two reviewers using the Cochrane RoB tool for randomized controlled trials (RCTs) and RoB for Nonrandomized Studies tool for non-randomized studies (NRSs) to reach a consensus. Data extraction was performed using the format provided by the subcommittee. Data analysis was performed by qualitative review. The Grading of Recommendations Assessment, Development, and Evaluation approach was used to determine the level of evidence of the systematic review results, and the grade of recommendation was based on the assessment results. The analysis results concerning the clinical safety and effectiveness of partial pulpotomy with MTA were first presented, and the conclusion was determined through a consensus.
Results
A total of five studies (four RCTs and one NRS) reported on MTA partial pulpotomy (hereinafter “intervention group”), all of which, compared this technique with partial pulpotomy using calcium hydroxide (hereinafter “control group”). A high RoB related to incomplete outcome data was found in 25% of the RCTs, with an uncertain RoB related to the comparability, confounding variables, assessor blinding, and selective reporting in one NRS.
Safety
One study found a pulp necrosis and infection rate of 14.3% (1/7) in the intervention group and of 5.4% (5/93) in the control group, with no statistically significant difference between both groups.
Accordingly, the subcommittee determined that the safety of partial pulpotomy with MTA is similar to that with calcium hydroxide.
Effectiveness
Two studies reported procedure success rates of 90% and 93% in the intervention group and of 93% and 91% in the control group, respectively, with no significant intra-group difference. One article reported a success rate of 85% in the intervention group and of 43% in the control group, showing a significantly higher success rate in the intervention group.
One study found a failure rate of 11.8% in both intervention and control groups, indicating no significant intra-group difference. Another study reported a failure rate of 15.3% and 56.5% in the intervention and control groups, respectively, with a significantly lower failure rate in the intervention group.
In one study, the tooth extraction rate was 4.55% in the intervention group and 2.5% in the control group, with no significant intra-group difference.
Accordingly, the subcommittee determined that partial pulpotomy with MTA is an effective technique and its success, failure, and tooth extraction rates are similar to those of partial pulpotomy with calcium hydroxide. However, the subcommittee also opined that there is a need for additional studies due to the low number of comparative studies included in the current assessment.
Conclusions and recommendations
The subcommittee recommends the following based on the current assessment results:
The safety and effectiveness of partial pulpotomy with MTA are similar to those of partial pulpotomy performed using calcium hydroxide. However, additional studies are necessary due to the low number of comparative studies included in the current assessment.
The 11th Health Technology Reassessment Committe in 2021 (November 12, 2021) concluded the following based on the subcommittee findings, in accordance with Article 4 Paragraph 10 of the Health Technology Reassessment Guideline:
The committee concurred with the opinion of the subcommittee that the safety and effectiveness of partial pulpotomy performed with MTA are similar to those of partial pulpotomy performed using calcium hydroxide, but there is limited evidence on its effectiveness due to the low number of comparative studies included in the current assessment.
In conclusion, the Health Technology Reassessment Committee decided that the safety and effectiveness of partial pulpotomy with MTA for patients with partial pulp infection or pulp disease is similar to that of partila pulpotomy performed using calcium hydroxide but there is limited evidence on its effectiveness, so it was deliberated on the conditional recommendation.
