연구의 목적
벤조다이아제핀계열 약물은 오남용의 가능성이 높기 때문에 과량처방의 잠재적인 가능성에 대한 우려가 있다. 과량처방에 대한에 관한 연구는 국내외에서 비교적 많이 이루어졌으나, 국내에서는 2006년까지 고령자를 대상으로 연구가 주로 수행되었으며, 벤조다이아제핀계열약물 사용과 관련된 부작용 연구는 거의 없었다. 따라서 국내에서 2006년 이후 고령자뿐만아니라 성인에서 벤조다이아제핀 사용현황 조사의 필요성이 있으며, 2010년 1월 이후부터 최면진정제 처방을 1회 30일로 제한하는 급여기준 변경 후, 처방패턴의 변화가 있는지에 대한 확인이 필요하다. 추가적으로 국내자료를 이용하여 벤조다이아제핀의 오남용으로 인한 부작용의 정도를 파악하는 것이 필요하다.
따라서 본연구의 목적은 첫 번째는 벤조다이아제핀계열 약물과 관련된 오남용 및 안전성 문헌고찰연구를 수행하여, 국내외 문헌고찰을 통하여 부적절한 벤조다이아제핀계열 약물 사용 및 오남용으로 인한 안전성 문제 및 경제적 부담을 파악하고, 국외 벤조다이아제핀계열 약물의 사용률을 비교하고자 한다. 두 번째는 벤조다이아제핀계열 약물의 국내 처방양상 조사을 조사하여, 우리나라 벤조다이아제핀계열 약물의 처방규모, 장기 사용률, 처방받은자의 주요 동반질환 및 오남용 현황을 제시하는 것이다. 세 번째는 벤조다이아제핀계열 약물과 관련된 안전성 성과연구로서, 국내에서 골절을 성과변수로 하여 벤조다이아제핀계열 약물과 관련된 안전성을 조사하는 것이다.
문헌고찰 연구
■ 연구방법
Embase 와 Ovid-Medline의 문헌검색database를 이용하여, 벤조다이아제핀계열약물에 대한 부적절한 사용(inappropriate medication use), 오남용 수준, 오남용으로 인한 안전성 문제 및 사회경제적 손실을 결과변수로 하는 문헌을 선택하였다. 2005년 1월 - 2012년 3월까지 Embase로부터 798편 Ovid-Medline를 통하여 3569 문헌을 찾아낸 후 중복을 제거하여 3879문헌을 선택하였다. 검색전략을 위하여 MeSH (Medical Subject Headings) 과 주제어가 사용되었다. 의료기술평가, 체계적문헌고찰, 메타분석, 경제학적 연구, 임상지침, 무작위 조절된 임상시험을 찾기 위한 검색식등이 이용되었다. 제목검토를 통한 1차 배제를 수행하여 591개의 논문을 선택하였고, 초록검토를 통한 2차 배체를 통하여 382개의 논문을 선택하였다. 382개의 논문은 연구주제에 따라서 원문을 검토하여서, 벤조다이아제핀의 사용/처방현황 (n=10), 지침(n=23), 차량사고(n=6), 자살(n=7), 인지기능장애(n=12), 골절 및 낙상(n=7), 임신 (n=3), 교육관련 중재방법 (n=7)을 주제별로 최종선택하였다. 지침과 교육관련중재방법 주제에는 Embase와 Ovid-Medline에서 선택된 문헌이외의 수기검색을 통하여 문헌이 추가되었다.
■ 연구결과
지역사회에서 전체연령을 대상으로 한 연구에서의 1년간 벤조다이아제핀처방 또는 사용을 한번이라도 한 적이 있는 인구의 퍼센트는 8.4-18.6%이었다. 15세 이상의 성인을 대상으로 한 연구에서는 3.9-14.5%이었고, 15-64세의 성인을 대상으로 한 연구에서는 3.3-9.5% 이었다. 65세 이상의 고령자를 대상으로 한 연구에서는 16.0-31.7%의 인구가 벤조다이아제핀을 한번이라도 처방 또는 사용하는 것으로 보고하였다. 연구방법으로 보험청구자료자료을 이용한 연구에 비하여, 인구집단에 대한 설문조사를 이용한 디자인을 이용한 연구에서 벤조다이아제핀사용이 낮은 경향을 보였다. (3.3%-3.9%)
벤조다이아제핀사용과 관련된 국가들의 지침을 보면 기분·행동장애·공항장애·수면장애에 대해서는 보조치료제로 단기사용을 권고하였고 노인에서는 장기사용 및 장기작용제제의 사용이 권고되고 있지 않았다. 치매의 경우도 행동 정신장애가 있는 경우는 권고되지 않았고 임상적 동요가 있는 환자에게 사용이 가능하나 부작용을 고려하여 용량조절의 필요성이 있고 단기작용제제가 선호되고 있었다.
벤조다이아제핀은 차량사고 및 낙상·골절의 위험을 증가시키는 것으로 보고되었고, 마약투약과 동반사용등이 자살위험을 높이는 것으로 보고되었다. 노인환자에서 벤조다이아제핀의 복용과 섬망발생과 같은 인지기능의 위험의 증거가 분명하지 않아서 더 많은 연구의 필요성을 진술하고 있으나, 대부분이 논문이 노인환자에서 벤조다이아제핀의 복용과 섬망발생과 같은 인지기능의 위험이 있을 수 있음을 언급하고 있다.
벤조다이아제핀계열약물의 오남용 사용을 줄이기 위해서는 급여기준에서 삭제 및 처방일 제한 등의 규제프로그램이 사용되었으나, 이러한 프로그램은 벤조다이아제핀의 사용량을 줄일 수 있으나, BZD의 적절한 사용에 대해서는 결론을 내리기가 어려우며, 벤조다이아제핀이 아닌 비슷한 고가의 다른 약제 처방이 증가하는 경향이 있었고, 정책의 변화가 연구집단 간에는 일관되지 않으며, 특히 만성적 정신질환이 있는 취약계층에서의 영향이 더 큰 경향을 보고 되었다. 처방 규제 프로그램이외 일차진료의사와 환자를 대상으로 한 교육 프로그램의 효과를 본 논문들은 이러한 프로그램이 벤조다이아제핀계열약물의 오남용 사용을 줄이는 데 효과적이라고 언급하고 있다.
건강보험심사평가원 청구자료 이용 벤조다이아제핀 계열 약물의 국내 처방양상 조사
■ 분석방법
건강보험심사평간원 (심평원) 청구자료의 요양개시일을 기준으로 2007년 1월 1일부터 2011년 12월 31일까지 5년간 의료기관을 방문하여 벤조다이아제핀 계열 약물을 1회 이상 처방받은 18세 이상의 환자 중 무작위로 5% 표본을 추출하여 연구 대상으로 하였다. 벤조다이아제핀 계열 약물은 WHO의 ATC Index, 심평원 약가주성분 코드, 심평원 수가파일, 복지부 분류코드에 근거하여 선정되었고, 속효성작용제제, 장기작용제제, 및 벤조다이아제핀 계열 유사약물(Z-drug)로 분류하였다. 벤조다이아제핀 약물처방의 환자사용량 분석을 위하여 WHO에서 규정한 성인에서 주요 적응증 치료를 위하여 사용되는 약물의 일일 평균 유지용량 (Defined Daily Dose, DDD)을 사용하여 분석되었다. 분석은 명세서 단위와 환자 단위 분석을 수행하였다. 환자단위 분석에서는 5% 추출 자료를 전국민 자료에서의 처방비율로 환산하기 위하여 20배의 가중치를 적용하였다. 하위그룹 (입원·외래, 성별, 연령별, 장기작용제제·속효성제제·Z-drug, 질환별)으로 벤조다이아제핀의 처방양상을 제시하였다.
■ 연구결과
명세서 단위의 분석 결과, 2007~2011년까지 연간 벤조다이아제핀 계열 약물로 처방된 명세서 (1,989,263건) 중 외래 및 입원에서 청구된 명세서는 94.6%와 5.4%이었다. 1개의 명세서는 평균 1.56개의 벤조다이아제핀 계열 약물의 성분을 포함하였으며, 처방된 약물건수는 장기작용 제제, 속효성 제제, Z-drug 순으로 52.6%, 40.6%, 6.7%였다. 성별 분류에 따른 명세서 처방건수는 여성 65.1% 남성이 34.9%였고, 의료기관 종별로 분류하면 77.1%가 의원에서 처방되었으며, 가장 많이 처방되는 성분은 장기작용 제제인 디아제팜이었다. 질환별로 분류하면 정신신경질환보다는 위·십이지장 질환으로 진단받은 경우가 가장 많았으며, 외래 명세서에 포함된 주·부상병 중 29.8% 입원 명세서 중 14.7%를 차지하였다.
환자 단위의 분석 결과, 2007년부터 2011년까지 5년간 1회 이상 벤조다이아제핀 계열 약물로 건강보험심사평가원에 청구된 18세 이상 환자 수는 총 22,361,449명이었다. 이러한 벤조처방 환자 모집단에서 연도별로 5% 무작위 표본 추출한 연구 대상자의 분석결과에 20배의 가중치를 곱한 후 연령 및 성별 전국민 인구수 100명 당 환자 수(%)를 추정하였다.
연간 18세 이상 인구 100명당 벤조다이아제핀 계열 약물을 1년에 1일 이상·30일 이상·90일 이상·180일 이상 처방받은 환자는 23.7%, 7.9%, 4.7%, 3.2%였다. 65세 이상에서는 46.0%, 23.3%, 15.2%, 10.7%이었다. 환자단위별 분석에서도 벤조다이아제핀 계열 약물은 남성보다는 여성, 65세 미만보다 65세 이상, 정신과보다 비정신과에서 더 많이 처방되는 경향을 보였고, 질환별로는 정신신경질환보다 비정신신경질환을 진단받은 벤조처방 환자가 많았다. 약물별로는 속효성 제제와 장기작용 제제가 비숫하게 처방되었고, Z-drug은 10%미만으로 처방되었다. 외래에서 벤조를 처방받은 65세 이상 환자에서는 장기작용 제제가 속효성 제제보다 약간 높았다.
벤조약물의 처방용량을 분석하기 위하여 주요 성분별 약물의 일일 평균 유지용량(DDD)과 1일 평균 처방용량을 비교하였을 때, 전반적으로 모든 벤조 약물을 일일 평균 유지용량보다 적게 사용하고 있었다. 연간 인구 천 명당 벤조다이아제핀 계열 약물의 하루 평균 DDD는 109.2였으며, 연간 벤조다이아제핀을 처방받은 환자 중 1DDD를 초과하여 처방 받는 환자는 10.1%였다. 벤조다이아제핀 처방환자의 연간 처방일수 분석결과, 65세 이상의 벤조처방 환자의 연간 평균 총 처방일수는 129일, 65세 미만은 62일로 65세 이상에서 처방일수가 2배 이상 길었다. 전체 인구 100명당 1.79명은 연속 30일을 초과하여 벤조다이아제핀 약물을 처방받은 적이 있었다.
65세 이상 장기작용 제제를 처방받은 환자 중 만성폐색성폐질환을 진단받은 비율은 남성과 여성에서 각각 10.6%, 5.5%였다.
연구기간 동안 벤조다이아제핀 계열 약물을 처음 처방받은 신환자(과거 1년 동안 벤조다이아제핀의 처방 이력이 없는 환자)는 연간 벤조처방 환자 중 56.6%였다. 벤조처방 신환자 중 1년에 90일과 180일 이상 처방받은 환자의 비율은 5.62%, 2.46%로 조사되었다.
건강보험심사평가원 청구자료 이용 성과연구
■ 분석방법
본 연구에서는 벤조다이아제핀계열 약물 사용이 골절발생에 미치는 영향을 살펴보기 위해 연구대상자 자신이 환자군인 동시에 대조군으로 활용될 수 있는 환자군만을 대상 (case-only)으로 한 디자인 중 환자교차연구(case-crossover design) 방법과 자신-대조 환자-씨리즈 연구 (self-controlled case-series design) 방법을 적용하였다 이러한 연구디자인들은 개인의 특성(성별, 유전적 감수성 등)으로 인한 교란효과를 통제할 수 있다는 장점을 가지고 있다. 골절의 진단코드를 가지고 응급실을 방문하거나 정형외과에서 처치 받은 대상자를 골절환자로 정의하였고, 교통사고나 뇌졸중 진단에 기인한 골절환자는 제외하였다. 환자교차설계 방법의 경우 conditional logistic regression 모형을 적용하여 오즈비(odds ratio, OR)와 95% 신뢰구간을 제시하였으며 자신-대조 환자-씨리즈 연구 방법의 경우 conditional poisson regression 모형을 적용하여 발생률비(incidence rate ratio, IRR)와 95% 신뢰구간을 제시하였다. 하위그룹분석으로서 벤조다이아제핀다디아제핀 계열 약물을 속효성제제, 장기작용제제, z-drug (zolpidem), alprazolam, diazepam으로 구분하여 세부분석을 수행하였으며, 성별 및 연령대별(65세 미만, 65세 이상) 추정치 및 95% 신뢰구간을 제시하였다.
■ 연구결과
환자교차설계 연구
연구 대상자는 2009년 벤조다이아제핀 계열 약물을 처방 받은 대상자 461,584명중에서 총 3,325명의 골절환자를 선정하였다. 150일전 대조기간을 기준으로 벤조다이아제핀을 처방받지 않는 경우에 비해 벤조다이아제핀을 처방받은 경우의 골절발생 오즈가 약 1.5배 높은 것으로 나타났다(OR=1.48, 95% C.I.: 1.16, 1.90). 다른 대조기간 시점(90일, 120일 전)에서도 오즈비의 차이는 크지 않았다.
하위그룹분석에서 65세 이상에서 장기작용제제사용 특히 디아제팜의 약물을 처방받은 하위그룹에서 벤조다이아제핀을 처방으로 인한 골절 발생의 위험의 오즈가 높았으며, 통계학적으로 유의하게 높게 나타났다. 벤조다이아제핀 계열 약물처방 유무에 따른 골절발생에 대한 오즈비(150일전 대조기간 기준)가 장기처방 받은 자(OR=1.24, 95% C.I.: 0.92, 1.66)에 비해 단기처방 받은 자(OR=2.32, 95% C.I.: 1.46, 3.70)에서 더 큰 것으로 나타났다.
자신-대조 환자-씨리즈 연구
연구 대상자는 2009년 벤조다이아제핀 계열 약물을 처음 처방받은 264,290명중 1개월간의 비노출기간을 설정하기 위하여 2009년 1월 한달 간 벤조다이아제핀을 처방받은 환자를 제외하고, 환자교차설계에서 정의한 골절이 발생한 6,623명의 환자를 대상으로 하였다. Conditional poisson regression 모형적합 결과를 살펴보면, 노출기간에서의 골절발생률이 더 높은 것으로 나타났으며(벤조다이아제핀 첫 처방~4주 기준 발생률비=1.87, 95% CI: 1.72, 2.03), 첫 벤조다이아제핀 처방일로부터 노출기간이 오래 될수록 발생률비는 점점 감소하는 경향을 나타내고 있었다. 하위그룹분석결과도 일관성을 보였다.
결론
벤조다이자제핀 계열 약물의 처방용량은 전체적으로 WHO에서 규정한 일일 평균 유지용량보다 적었으나, 처방은 매우 빈번하게 이루어지고 있었다. 18세 이상 성인인구 100명당 벤조다이아제핀 계열 약물의 1일 이상·30일 이상·90일 이상·180일 이상 처방받은 환자 수는 23.7%, 7.9%, 4.7%, 3.2%였다. 특히 65세 이상의 고령자에서 더 빈번한 처방이 이루어졌고 2007년부터 2011년 사이에 처방패턴이 변화하지는 않았다. 벤조다이아제핀 처방은 의원급 외래에서 많이 이루어지고 있었고, 장기작용제제인 디아제팜처방이 많았고, 정신신경계질환보다 소화기질환에서의 처방이 더 많았다. 심평원 청구자료를 이용한 분석에서 골절과 벤조다이아제핀 처방과의 상관관계를 확인할 수 있었다. 특히 65세 이상의 장기작용제제를 처방받은 환자에게서 주의가 요구되며 이 약을 처음 복용하는 환자들에 대한 주의가 필요하다. 벤조다이아제핀의 처방을 규제하기 위해서, 강제 규제프로그램의 도입은 외국사례를 참고할 때, 유사한 고가약의 처방을 증가시키거나 만성정신질환자나 소득이 낮은 취약계층에세 더 영향을 미칠 가능성이 있다. 현재 시행되고 있는 향정신성의약품 중복처방 제한 및 DUR시행 (2011년)과 더불어, 벤조다이아제핀의 적절한 사용을 위해서는 외국에서 효과를 보이고 있는 일차진료의 및 환자를 대상으로 한 교육프로그램의 도입에 대한 타당성에 대한 검토가 필요하다.
Objective
There were concerns regarding the potential over-prescription of Benzodiazepines (BZD) due to their nature for dependency and addition. Prescription patterns and safety related issues for BZD were studied in several countries. But, there were few recent prescription pattern studies and the safety related with BZD use were rarely studied in Korea. Since January, 2010, the limitation of ≤30-day supply for sedative hypnotics including BZD was implemented in Korea due to the possibilities of their inappropriate use. Assessment whether this policy that decreased the use of BZD would be required in terms of public health aspects.
The objective of this study is as follows.
-To review literatures on inappropriate medication use, safety, and social-economic costs related with BZD use
-To investigate prescription patterns of BZD in South Korea between 2007 and 2011
-To investigate the association between fracture and BZD presciption in South Korea
Literature review
■ Methods
Using the database of Embase and Ovid-Medline, literatures on inappropriate medication use, safety, and social-economic costs related with BZD use were searched. The search strategy comprised controlled vocabulary such as MeSH (Medical Subject Headings) terms and keywords. Methodological filters were applied to restrict the study types such as health technology assessments, systematic reviews, meta-analysis, economic studies, guideline, and randomized controlled studies. From Jan, 2005 to March, 2012, 798 literatures from Ovid-Medline and 3569 literatures from Embase were selected. After removal of duplicated literatures, 3879 literatures were selected. Firstly, 591 literatures were selected through the title review, and then 382 literatures were selected through the review of abstract. In reviewing of full texts, prescription/treatment pattern (n=10), clinical practice guidance (n=20), motor vehicle accidents (n=6), suicide (n=7), cognitive impairments (n=12), fall and fracture (n=7), pregnancy (n=3), and interventions related with BZD appropriate use (n=7) were finally selected.
■ Results
The percentage of people who filled BZD prescription of at least once was 8.4-18.6% during one year in all population without restriction of age. It was 3.9-14.5% and 3.3 -9.5% among adult population of >= 15 years old and of 15-64 years old, respectively. Among the elderly of 65 years old or more, it was 16.0-31.7%. The percentage of people who filled BZD prescription was lower using survey methods than claim database.
All the clinical practice guideline recommended that BZD were not recommended as the first line treatment for the mood and behavioral disorder, panic disorder, and insomnia. In particular, they recommended that health professionals should be cautiously prescribed BZD for the elderly. The use of long acting BZD and long term use were not recommended for the elderly. For the treatment dementia, the use of BZD may produce the benefits to reduce the psychotic symptoms and agitation/aggression that outweigh the risk of adverse events such as fall and confusion. The use of short-acting BZD and the dose adjustment were recommended for dementia patients.
The use of BZD increased the risk of fall/fracture and motor vehicle accidents. When BZD used concomitantly with opioid, the risk of suicide commitment increased. In few article said that the evidence for the association between delirium and BZD use were not sufficient. But, most of article reported the higher possibility of adverse events of delirium related with BZD.
In western countries, numerous strategies (i.e. exclusion of BZD from reimbursement list or limitation of 30-day supply) related with the restriction of BZD prescription were implemented. But, these policies led to a moderate decrease of BZD prescription, but it was likely to increase alternative medicines such as serotonin-specific reuptake inhibitor (SSRI) or tricycle antidepressants and affected more for vulnerable people who were lower socio economic status and chronic mental health disease. In addition, several articles reported that educational program for general practitioners or patients improved appropriate use of BZD.
Prescription Trends of BZD using Health Insurance Review and Assessment (HIRA) Database
■ Methods
We selected study subjects by 5% random sampling from adult patients (≥18 years old) who visited clinics or hospitals and have prescription of BZD from Jan.1, 2007 to Dec.31, 2011 using HIRA database. BZD was defined using WHO ATC Index, HIRA drug master file, and etc and was grouped by short-acting, long-acting, and BZD like drug (Z-drug). Defined Daily Dose(DDD) by WHO was used to analyze the data in terms of dose and periods of BZD prescription. Data analysis was performed by claim unit or patient unit. For the analysis of patient unit, each claim was merged by same patient. In patient unit analysis, weight value of 20 was multiplied in order to convert 5% sampling data into national data. Prescription patterns of BZD were presented by subgroup of inpatient vs out-patients, long-acting vs. short-acting vs. Z-drug, and indication for BZD prescription.
■ Results
Data analysis by claim-unit base
Among total BZD claim of 1,989,263, the percentage of BZD claims for inpatient and out-patients was 94.6% and 5.4%, respectively. One claim included average 1.56 active ingredients of BZD. The percentage of claim for short-acting, long acting, and Z-drug was 52.6%, 40.6%, and 6.7%, respectively. When BZD claims were classified by sex, the percentage of claim for women and men was 65.1% and 34.9%. Clinics were most frequent provider for BZD prescription (77.1%) when medical centers were divided by clinics, hospitals, and general hospital. Most frequent prescribing active ingredient was diazepam, and gastro-duodenal diseases was the most frequent indication for BZD prescription. (29.8% among out-patient claims and 14.7% among in-patient claims)
Data analysis by patient-unit base
BZD prescription of at least once during five years occurred for a total of 22,361,449 patients (≥ 18years old) using 5% sampling data. The average national prescription prevalence per 100 people per one year was 23.7%, 7.9%, 4.7%, and 3.2 % for BZD annual prescription of ≥1day, ≥30days, ≥ 90days, and ≥ 180days, respectively using total number of population (≥18 years old) during study periods. Among elderly people of ≥ 65 years old, 46.0%, 23.3%, 15.2%, and 10.7%, respectively. Subgroup analysis was similar to claim data analysis. Clinics were most frequent provider of BZD prescription and the prescription prevalence was higher in the elderly (≥65 years old) than younger people (<65 years old), women than men, non-psychiatric department than other departments, and non-mental disease than mental disease. The frequency of prescription of short acting and long acting were similar, and Z-drug was prescribed to subjects of <10%. Among the elderly in an out-patient setting, long-acting was slightly higher than short acting BZD. In terms of one day dosage, overall BZD dosage was lower than average one day dose defined by WHO. DDD per 1000 population per one day was 109.2. Among the subjects who were prescribed to BZD, the subjects with the prescription of 1 DDD or more were about 10%. The average prescription days per one year were 129days for the elderly and 62days for younger subjects (18-64 years old) Among 100 population (≥18 years old), 1.79 subjects had BZD prescription of continuous 30-day supply or more. Among the elderly who were prescribed to long-acting BZD, those who have COPD diagnosis were 10.6% for men and 5.5% for women. The subjects with new BZD prescription (defined as the subjects who had no BZD presciption over previous one year) were about 56.6% among people with BZD prescription. The subjects who had BZD prescription over 90-day supply and 180-day supply were 5.62% and 2.46% among subjects with new BZD prescription, respectively.
Outcome study using Health Insurance Review and Assessment (HIRA) Database
■ Methods
This study used case-crossover design and self-controlled case-series design to investigate the association between fracture and prescription of BZD. These study designs used case only and had merit to control confounding bias such as individual medical and demographic characteristics. Patients with fracture was defined that patients visited emergency room and orthopedics with ICD-10 diagnosis code of fracture. If they had fracture due to the motor vehicle accident and stroke, they were excluded. Whereas case-crossover design presented Odds Ratio(OR) and 95% Confidence Interval (CI) using conditional logistic regression model, self-controlled case-series design showed incidence rate ratio(IRR) and 95% CI using conditional poisson regression model. Subgroup analysis by short-acting vs. long-acting vs. Z-drug and the kind of active ingredient were conducted. The OR and IRR value including 95% CI were presented by sex and age group.
■ Results
Case-crossover design
A total of 3,325 patients with fracture was selected among 461,584 people who were prescribed to BZD in 2009. ORs for fracture among people who were prescribed to BZD were about 1.5 times higher than people who were not prescribed to BZD. Using control period of before 150-151 days from case periods, OR was 1.48 (95% C.I.: 1.16, 1.90). Sensitivity analysis using different control periods (before 90-91 days and before 120-121 days from case development) showed similar results. The elderly who were prescribed to long-acting BZD showed higher OR than other groups. Patients with short-term prescription had higher ORs than people with long-term prescription. (OR=1.24, 95% C.I.=0.92-1.66 for long-term vs. OR=2.32, 95% C.I.=1.46-3.70 for short term).
Self-controlled case-series design
A total of 6,623 patients with fracture was selected from 264,290 people who were newly prescribed to BZD in 2009. When patients who had BZD prescription during one month of Jan, they were excluded to put non-exposure period of one month. IRR for fracture among people who were prescribed to BZD were 1.87 times (95%CI=1.72-2.03) higher during initial four week periods than non exposure periods. IRR was lower when exposure periods were longer. Subgroup analysis was in accordance with the main results.
Conclusion
Overall BZD dosage was lower than WHO defined daily dose, but BZD prescription was frequent. The average national prescribing prevalence per 100 people per one year was 23.7% for ≥1day, 7.8% for ≥30days, 3.2% for ≥ 180days, respectively using total number of population (≥18 years old) during study periods. BZD prescription was much frequent among elderly people of ≥ 65 years old and one-year prescription trend was similar during five years of study periods. BZD prescription much occurred at out-patient visit in clinics than other types of medical centers. When they were classified into kind of active ingredient, the prescription of diazepam , one kind of long-acting BZD, was most frequent. BZD was prescribed more for the indication of gastro-duodenal disease than neuro-psychological diseases. The risk for fracture was higher in patients with BZD prescription than people without BZD prescription. In particular, elderly people who were 65 years old and were prescribed to long-acting BZD had higher OR than other groups. The people with short-term BZD prescription or exposure in early period from BZD initial prescription had trends of higher risk for fracture than those with long-term or exposure in late period.
From the lesson on the experience of western countries, policy implementations such as limitation of supply periods and exclusion from reimbursement list were likely to increase alternative medicines and affected more for vulnerable people who were lower socio economic status and chronic mental health disease. The validity for educational program for general practitioners or patients which was reported as effective measures in other countries would be investigated for appropriate use of BZD.
