1. 서론
2009년 WHO에서 권고안을 통해 9-13세의 모든 여성에게 HPV 예방 접종을 권고한 이후 미국, 유럽, 호주 등지에서는 국가 차원에서 HPV예방접종을 도입하거나, 국가 또는 지역자치단체차원의 실정에 따라 재정지원을 하고 있는 추세이다. 한국에서 시판되고 있는 HPV 백신은 가다실(Gadasil, Merck&Co.,Inc)과 서바릭스(Cervarix, GSK)가 있다. 가다실은 2007년 6월, 서바릭스는 2008년 7월부터 식품의약품안정청(KFDA)1)으로부터 시판허가를 받고 도입되었다. 아직 우리나라에서는 HPV 백신이 국가예방접종으로 시행되고 있지 않으며 국내 관련 학회에서 권고하는 접종대상에서도 차이가 존재하고 있다.
한국에서 수행된 선행연구 모두 비용-편익 분석으로 편익에 비해 비용이 높은 것으로 나타나 현재 백신 가격 수준에서는 경제성이 없는 것으로 결론지었다. 그러나 자료의 부족으로 분석 단계에서 여러 가정을 사용하는 등 연구의 불확실성이 큰 것으로 보고되었다. 본 연구는 HPV 백신의 경제성을 평가함에 있어 보건의료분야에서 더 많이 활용되는 비용-효용 분석을 적용하였다. 이를 위해 건강보험청구자료 분석 및 환자 조사 등을 통해 연구의 완결성을 높이고자 하였다. 본 연구의 목적은 HPV 백신의 경제성 분석을 통해 관련 정책 결정 및 의사 결정에 근거를 제공하는 것이다. 사회적 관점에 따른 HPV 백신의 경제성 분석 결과를 통해 국가 예방접종프로그램 도입 여부에 대한 합리적인 의사결정에 기여할 것으로 예상한다.
2. HPV 감염 역학
2.1. 연구방법
성행동 패턴, 백신 접종률, HPV 감염률 및 HPV 유형, CIN(cervical intraepithelial neoplasia)과 침윤성 자궁경부암에서의 HPV 유형 분포, 자궁경부암 전암 상태와 침윤성 자궁경부암의 질환통계, 자궁경부암의 생존율 등에 관련된 내용을 문헌 고찰하였다. 경제성 분석에서 활용하기 위해 역학 자료는 한국인을 대상으로 한 연구를 우선적으로 살펴보았으며, 국내 자료가 없거나 국내 자료와 비교를 위해 국외 문헌도 선택적으로 확인하였다.
2.2. 연구결과
한국인의 성행동 패턴에 대한 문헌은 김찬주 등(2007), 신경림 등 (2010), 제6차(2010년) 청소년건강행태온라인조사 통계 자료가 있다. 지표 정의가 달라 직접적인 비교는 어렵지만, 검토결과 우리나라의 젊은 여성들에서 첫 성관계가 이루어지는 평균연령은 미국, 유럽, 호주 등에 비해 높으면서 성관계 경험률은 낮은 것으로 나타났다. 첫 성관계가 이루어지는 평균연령은 19-21세로 보고되었으며 6대 대도시(서울, 부산, 대구, 인천, 광주, 대전)에 거주하고 있는 12-29세 여성을 대상으로 한 조사에서는 첫 성관계를 경험한 나이가 만 16세 이하인 비율은 5%인 것으로 나타났다.
한국의 HPV 백신 접종률에 대한 공식 통계는 없다. 단, 서울 소재 5개 대학의 여학생 200명을 대상으로 조사한 결과 12%가 접종을 한 것으로 보고한 문헌이 있다(방경숙 등, 2011).
한국의 HPV 감염 유병률은 약 10-15%로 추정되며 특히 젊은 연령에 서 높게 나타났다(신해림 등, 2007). 특히, 젊은 여성 중 성경험이 있는 여성에서 HPV감염률이 38.8%로 일반여성 15.2%에 비해 높았다(신해림 등, 2007). 15개국의 성경험이 있는 일반 여성을 대상으로 연령별 HPV 감염률을 보고한 Franceschi 등(2006)에서는 아프리카가 27%로 HPV 감염률이 가장 높았고, 유럽이 2.7-9.4%로 가장 낮았으며, 한국은 14.8%로 중간 정도에 해당하였다. 한국 여성을 대상으로 체계적 문헌고찰(Systematic Review)을 수행한 결과 HPV 유형 중 감염률이 가장 높은 유형은 HPV 16번 유형으로 6%의 감염률을 보였으며 그 다음이 HPV 58번, HPV 18번 유형 순이었다(Bae 등, 2008).
자궁경부암 전암 상태(ASCUS, CIN 1) 환자에서의 HPV 감염률은 63.2%였으며, 고위험 HPV 감염 비율은 56.3%였다. CIN 2, 3 또는 CIS 환자에서의 HPV 감염률은 85.6%로 고위험군 HPV 감염비율은 83.7%였다. 자궁경부암 환자에서의 HPV 감염률은 88.3%였고, 고위험군 HPV 감염비율은 84.6%인 것으로 확인되었다. 우리나라가 속한 동아시아 연구결과를 비교했을 때 아시아 전체의 결과와 호발 유형 5위까지 는 비슷한 경향을 보였다(Bae 등, 2008).
중앙암등록자료(2009)에서 한국의 자궁경부암 신환자는 1999년에는 4,443명, 2009년에는 3,733명이었으며 연도별 연령표준화 발생률 또한 1999년 18.6명에서 2009년 12.0명으로 감소하였다. 한국 여성이 평균 수명 84세까지 생존한다고 가정했을 때 평생에 한 번 이상 자궁경부암에 걸릴 확률은 1.4%였다. 자궁경부암 생존율은 국내 보고가 적어 우리나라 자료도 포함된 FIGO Annual Report를 활용하였다. 전체 자궁경부암 11,775명을 대상으로 한 전체 생존율 분석에서 자궁경부암 환자의 평균 연령은 51.7세였으며 1년 생존율은 89.8%, 2년 80.7%, 3년 75.5%, 4년 72.3%, 5년 69.3%로 보고하고 있다. 한편 무진행 생존율은 1년 92.9%, 2년 85.4%, 3년 81.4%, 4년 79.3%, 5년 77.7%로 나타났다(Quinn 등, 2006).
3. HPV 백신의 임상적 효과성 평가
3.1. 연구방법
체계적 문헌고찰을 통해 HPV 백신의 임상적 효과성 및 안전성에 대한 최신의 근거를 확인하고자 하였다. 환자군은 여성을 대상으로 모든 연령대의 대상자를 포함하였으며 인종, 지역, HPV 감염여부 및 성경험 여부 등에 제한을 두지 않았다. 중재법은 HPV 백신을 모두 포함하였다. 주요 결과로 HPV 16, 18번 관련 CIN2+ 및 6개월/12개월 지속감염률(persistent-infection rate) 등을 확인하였다. 검색 DB는 국내 DB로 KoreaMed, KISS, KMBASE, NDSL를, 국외 DB는 Ovid-Medline, Ovid-EMBASE, CENTRAL(Cochrane Library)을 검색 하였다. 1차 선택/배제 과정과 2차 선택/배제 과정을 거쳐 선정된 문헌들에 대하여 Cochrane’s Risk of Bias(RoB)를 사용한 비뚤림 위험 평가를 실시하였다. 선택된 ITT(intent-to-treat), mITT(modified-intent-to-treat), PP(per protocol) 기준에 따라 각 문헌에서 보고된 임상시험 결과를 분류하여 메타 분석을 수행하였다.
3.2. 연구결과
문헌 5,661건 중 총 9개의 임상연구를 선정하였다. 각 연구의 비뚤림 위험은 전체적으로 낮은 경향을 보여 결과에 영향을 주지 않을 것으로 판단되었으며, 각 임상연구의 결과 제시 방법에 따라 ITT, mITT, PP분석으로 나누어 분석하였다.
HPV 16, 18번 유형과 연관된 CIN 2+ 결과를 메타분석 결과 ITT분석에서는 RR이 0.52[95% CI 0.37-0.7]로 분석되었다. mITT분석에서는 RR이 0.09[95% CI 0.04-0.22], PP분석에서는 RR이 0.06[95% CI 0.03-0.12]으로 모든 분석에서 백신군에서 16, 18번 유형과 연관된 CIN 2+를 유의하게 감소시키는 것으로 분석되었다(그림 1).
즉, 감염여부와 관계없이 한 번 이상 예방접종을 받은 모든 사람을 대상으로 본 효과라고 할 수 있는 ITT분석의 RR값은 0.52로 백신의 16, 18번 유형과 연관된 CIN 2+에 관련한 임상적 효과는 48%로 추정할 수 있다. 기존 감염자를 배제하고 한번 이상 예방접종을 받은 사람을 대상으로 분석하는 mITT 분석에서 CIN 2+관련 임상적 효과는 91%이었으며, 감염이 되지 않은 사람들 중 3회 모두 접종한 사람들을 대상으로 한 PP분석에서는 16, 18번 유형 관련 CIN 2+ 관련 임상적 효과는 94%로 볼 수 있다.
HPV 16, 18번의 6개월 지속 감염률은 ITT와 mITT를 통합 분석한 결과 HPV 백신은 6개월 지속감염률을 56%(RR=0.44[95%CI 0.34-0.57]) 낮추는 것으로 나타났고, PP분석에서는 92%(RR=0.08[CI 0.04-0.15]) 감소시키는 것으로 확인되었다. 12개월 지속 감염률의 경우 TT와 mITT를 통합 분석한 결과에서는 RR 0.45[CI 0.34-0.58]였으며, PP분석에서는 0.09[CI 0.06-0.13]로 분석되었다(그림 2).
교차 보호 효과(cross protection effect)에 대해서는 포함된 연구수가 적고 4가 백신만 포함되어 결론을 위해서는 좀 더 많은 연구를 검토할 필요가 있을 것으로 생각된다. 현재까지의 백신 관련 임상시험에서의 안전성 결과나 시판 후 분석 등의 결과를 살펴보았을 때, HPV 백신이 안전하지 않다고 볼 근거는 없었다. 따라서 현재까지의 자료로는 안전하다고 판단할 수 있으나, 장기간의 안전성, 시판 후 분석 결과를 관찰할 필요가 있다.
4. HPV 감염 관련 질환의 환자수 및 의료비용
4.1. 연구방법
HPV 감염 관련 질환의 환자수 및 의료비용 추정을 위해 건강보험심사평가원에서 제공하는 건강보험청구자료를 활용하였다. 2007년 1월 1일-2011년 12월 31일까지 5개년도의 요양개시일을 기준으로 주상병을 포함한 제 5부상병에 HPV 감염 관련 질환의 상병코드가 있는 경우를 자료원으로 활용하였다.
유병환자 파악에 있어 해당 질환 상병코드가 주상병 또는 제1부상병에 기입된 경우로 정의하였으며 암 환자의 경우 명세서 내역 정보(T200)의 특정기호 구분코드 중 암 관련 코드(V027, V193, V194)가 함께 코딩된 환자로 제한하였다.
경제성 분석 모형에서 고려한 자궁경부암 질환인 CIN 1, CIN 2/3, 자궁경부암에 대해서는 더 자세한 분석을 수행하였다. 신환자는 과거 2년간 해당질환으로 의료이용이 없었던 환자로 정의하였으며 이를 위해 2007-2008년 자료를 과거력 제외기간으로 활용하였다. 임상의의 자문을 받아 상병코드 N870은 CIN 1, N871은 CIN 2, N872 및 D06은 CIN 3로 정의하였다. 그러나 N87의 하부코드가 없거나 N879와 같은 미분류된 상병코드가 50% 수준이어서 이를 추가로 고려한 CIN 신환자수도 함께 산출하였다. 자궁경부암(C53)은 건강상태를 좀 더 세분화하여 진단 후 2년째부터 추적 관리를 하거나 재발 또는 지속(recurrent/persistent) 상태에 있는 경우를 구분하여 분석하였다. 이를 위하여 임상 전문가의 자문을 받아 2년차, 3년차에 수술치료, 항암치료, 방사선치료를 받은 환자를 재발/지속환자로 정의하였다. 수술치료, 항암치료, 방사선치료의 처치 코드에 대해서도 임상의의 도움을 받아 건강보험심사평가원의 청구코드와 연계하여 활용하였다.
4.2. 연구결과
(1) 자궁경부암(C53)의 유병률과 의료비용 2011년 인구 10만명당 자궁경부암 유병률은 90.8명으로 최근 들어 감소추세를 보이고 있다. 연령별로는 40-60대 여성에서 주로 발생하는 것으로 나타났으며, 60대 여성 10만명당 자궁경부암 유병률은 194.9명이었다. 자궁경부암(C53) 환자의 1인당 평균 요양급여비는 3,482,030원으로 연도별로 유병 환자수는 감소하는데 비해 1인당 요양급여비는 꾸준히 증가하는 것으로 나타났다.
(2) 자궁경부암(C53) 신환자의 발생률, 재발비율, 의료비용
CIN 1 발생 환자수는 미분류 CIN 환자 고려 여부에 따라 2009년 25,946명(10만명당 발생률 104.4명)에서 46,463명(10만명당 발생률 187.0명) 수준이었으며 2011년 35,018명(10만명당 발생률 139.9명)에서 69,431명(10만명당 발생률 277.4명)으로 계속 증가하는 경향을 보이고 있었다. CIN 2/3 환자의 경우 10만명당 발생률이 추정방법에 따라 2009년 97.8명-175.1명 수준이었으며 2011년은 88.1명-174.8명으로 감소하였다.
자궁경부암의 신환자수는 2009년 5,593명(10만명당 발생률은 22.5명)에서 2011년 4,958명(10만명당 발생률 19.8)로 감소하는 경향을 보였다. 연령별로 증가하다 70세에서 정점을 이루는 것으로 확인되었다(그림 3).
자궁경부암 신환자의 재발비율을 분석한 결과 2008년 하반기 신환자 중 2년째에 재발/지속 환자는 5.44%로 나타났다. 발병 3년차에 추적관리를 하다 재발한 사람은 1.8%, 재발 후 다시 재발/지속상태인 환자는 25.8%였다. 2009년 기준으로 했을 때 2년차 재발/지속환자 비중은 6.42%였다. 1인당 평균 의료비용을 추정하기 위해 이상치 ±3%를 제외하고 1인당 평균 요양급여비를 분석한 결과 2009년도 CIN 1 신환자의 1인당 평균 요양급여비는 209,979원으로 CIN 2/3 환자의 1인당 평균 요양급여비는 679,291원으로 CIN 1 환자에 비해 3배 이상이었다. 2009년 자궁경부암 신환자의 1년차 1인당 평균 요양급여비는 657만원이었고 재발이 없이 추적관리 환자의 1인당 평균 요양급여비는 100만원, 재발한 환자는 1,622만원으로 재발환자에서 비용이 16배 더 높았다.
5. 자궁경부암 관련 질환 비용조사
5.1. 연구방법 자궁경부암의 비용을 파악하기 위해 한국보건의료연구원 및 각 의료기관의 연구윤리심의위원회(IRB)의 승인을 받아 서울 및 경기도 수도권 내 6개의 의료기관에서 자궁경부암 환자의 비용자료를 수집하였다. 비용자료는 참여 의료기관에서 제출한 2009년 CIN 또는 자궁경부암을 진단받은 신환자의 3년간 진료비계산서 자료를 이용하여 후향적으로 조사하였다.
5.2. 연구결과
6개 의료기관에서 2009년 발생한 자궁경부암 관련 비용자료를 수집한 대상자는 총 1,705명이었다. 이 중 질병이 발생한 첫 해 의료이용이 없었던 환자 17명을 제외하고 1,692명을 최종 분석하였다. CIN 1 환자는 288명이었고, CIN 2/3환자는 675명이었다. 자궁경부암 환자는 729명이었고 이중 자궁경부암이 재발하지 않은 환자는 655명(89.8%)이었다.
CIN 1 환자의 진단 1년차 1인당 평균 요양급여비는 약 56만원이었고 총 의료비는 약 85만원이었다. 비급여 본인부담금은 약 29만원이었으며 법정 본인부담금을 합한 본인부담금 총액은 약 51만원이었다. 총 의료비 중 본인부담금의 비중은 1인당 평균 72.5%였고, 비급여 비중은 1인당 평균 37.5%였다. CIN 2/3 환자의 진단 1년차 1인당 평균 요양급여비는 약 82만원이었고 총 의료비는 약 120만원이었다. 비급여 본인부담금은 약 37만원으로 본인부담금 총액은 약 63만원이었다. 총 의료비 중 본인부담금의 비중은 1인당 평균 58.5%였고, 비급여 비중은 1인당 평균 35%였다.
자궁경부암 환자 중 재발하지 않은 환자의 1년차 1인당 평균 총 의료비는 1,840만원, 2년차 총 의료비는 170만원, 3년차 총 의료비는 120만원이었다. 재발하지 않은 환자에서는 연도별로 의료비용이 점차 감소하였으며 총 의료비 중 본인부담금 비중과 비급여 비중은 연도별로 큰 차이가 없었다. 자궁경부암이 재발안한 환자의 입원횟수와 입원일수, 외래횟수는 연도별로 감소하였다. 재발한 환자의 재발 1년차 1인당 평균 총 의료비는 3,000만원, 재발 2년차 총 의료비는 656만원, 재발 3년차 총 의료비는 165만원이었다. 재발한 환자의 연도별 의료비용은 재발 1년차가 가장 높았으며 총 의료비 기준으로 1년차 대비 2년차 비용은 78.6% 감소하였고, 2년차 대비 3년차 비용은 74.8% 감소하였다.
6. 자궁경부암 관련 질환 대상 환자 설문조사
6.1. 연구방법
자궁경부암 환자를 대상으로 삶의 질 등을 측정하고 질환과 비용 산출을 위한 관련 자료를 얻고자 한국보건의료연구원 연구윤리심의위원회(IRB) 승인 및 각 의료기관의 IRB의 승인을 받아 서울 및 경기도 수도권 내 6개의 의료기관에서 설문조사를 수행하였다. 조사대상자는 CIN 또는 자궁경부암으로 각 의료기관 산부인과를 한 환자를 대상으로 하였다.
설문조사는 2012년 10월-12월까지 진행되었으며 총 설문조사 참여자는 452명이었다. 환자의 삶의 질은 EQ-5D 도구를 활용하여 측정하였고, 주요 조사내용은 환자 기본정보, 삶의 질, 비공식적 의료비용, 시간비용 및 간병비용 등으로 구성되었다.
6.2. 연구결과
환자 설문조사 결과 삶의 질 가중치는 CIN 1 환자 0.937, CIN 2/3 환자 0.933, 자궁경부암 환자 0.874, 자궁경부암 재발환자 0.784로 중증일수록 삶의 질이 낮았다.
자궁경부암 환자(자궁경부암 재발환자 포함)의 경우 초기 1년 동안의 비공식적 의료비용이 가장 높았으며, 치료기간이 길어질수록 비공식적 의료비용은 감소하는 것으로 나타났다. 자궁경부암 환자의 질병의 상태가 중할수록, 치료기간이 짧을수록(진단일로부터 1년 이내) 입내원일수, 간병일수, 간병시간이 길었다. 의료기관 방문시 보호자 및 유급 간병인 동행여부를 통해 간병률을 추정한 결과 입원/외래 모두 CIN 환자에 비해 자궁경부암 환자(재발포함)의 간병률이 월등히 높았으며, 발병 후 1년 미만 환자의 간병률이 가장 높았다. 자궁경부암 환자가 진료를 위해 외래 방문시 소요되는 평균시간은 CIN 1 환자 1.7시간, CIN 2/3 환자 4.2시간, 자궁경부암 환자 4.7시간, 자궁경부암 재발환자 5.6시간으로 확인되었다.
7. HPV 백신의 경제성 분석
7.1. 연구방법
(1) 모형의 개요 국가 예방접종프로그램으로 12세 여아에게 HPV 백신 접종을 하는 대안과 현재의 자궁경부암 검진만 하는 대안에 대한 경제성 분석을 수행하였다. HPV 백신 접종으로 HPV 감염의 감소가 CIN과 자궁경부암 발생 감소에 미치는 영향을 확인하였으며 최종적으로 수명연장과 삶의 질을 고려한 질보정수명연수(QALYs)를 적용하여 비용-효용 분석을 수행하였다. 사회적 관점에서 의료비용 뿐 아니라 시간비용, 간병비용, 교통비용 등을 모두 포함하였다. 분석주기를 1년으로 하고 분석기간을 평생으로 한 마콥 모형을 적용하였다. 모형에서 고려한 건강상태는 건강(well), HPV 감염, CIN 1, CIN 2/3, 자궁경부암(initial cancer), 자궁경부암 추적 관리, 재발암 또는 지속암(recurrent/persistent cancer), 재발암/지속암 추적 관리, 사망으로 구분된다. 이때 HPV 감염은 HPV 16, 18번 유형 특이적으로 적용하였다(그림 4).
예방접종이 자궁경부암 검진율에 영향을 주지 않으며 백신의 보호효과는 평생 유지되는 것으로 가정하여 분석을 수행하였다. 또한 집단면역 효과, 교차보호효과 등을 고려하지 않았다.
(2) 주요 전이확률 HPV 감염 발생확률은 연령별 HPV 유병률, 성경험률, 감수성 및 HPV 감염 소실률 등을 고려하여 시뮬레이션(calibration)을 거처 추정하였다. CIN 치료 및 관리 비중은 우리나라 여성들의 연 평균 자궁경부암 검진비율을 적용하였으며 CIN 1에서 CIN 2/3, CIN 2/3에서 자궁경부암으로의 전이확률은 건강보험청구자료 분석 결과를 활용하였다. 자궁경부암 각 상태별 사망확률과 전이확률을 확인하기 위해 FIGO annual report(Quin 등, 2006), 건강보험청구자료 분석, 선행 연구 등을 활용하였다.
(3) 비용 백신 비용의 경우 국내에서 시판되고 있는 HPV 백신인 가다실과 서바릭스의 비중을 고려하여 기본 분석에서는 두 가지 백신 비중을 각각 50%로 하고, 국가 예방접종 프로그램으로 도입될 경우 시장 가격보다 70% 할인되는 것으로 가정하여 343,144원을 적용하였다. 검진비용은 국가 암 검진과 민간검진의 비율 및 연령에 따른 검진율을 적용하여 산출하였다. CIN 및 자궁경부암으로 진단을 받은 환자의 연간 의료비용은 건강보험청구자료 분석 결과를 활용하였고 비공식적 의료비용은 환자 설문조사 결과를 적용하였다. 건강보험청구자료 분석과 환자 설문조사 결과를 이용하여 의료이용량을 파악하고 선행연구에서 보고한 단위비용을 적용하여 교통비용, 시간비용, 간병비용을 산출하였다.
(4) 효과 체계적 문헌고찰을 통해 메타분석한 HPV 16, 18번 유형의 12개월 지속감염률을 활용하였다. 본 연구에서는 1회 이상 접종한 사람의 비율을 이용하여 효과를 적용하기 위하여 mITT 분석 결과인 RR 0.45[CI 0.34-0.58]를 적용하였다.
(5) 민감도 분석 변수의 불확실성 및 변동 가능성이 있는 부분을 반영하기 하기 위하여 민감도 분석을 수행하였다. 백신의 가격 및 인하율, 예방접종 비율, 백신의 효과에 대해 민감도 분석을 수행하였고 그 외에도 결과에 영향을 줄 수 있는 의료비용, 자궁경부암 재발 환자에서의 사망률 변화에 따른 결과를 확인하였다.
7.2. 연구결과
(1) 총 효과 12세 여아 코호트에서 HPV 예방접종 프로그램 도입시 HPV 백신 접종 프로그램군의 자궁경부암 발생환자 수는 2,042명이었고 현재 검진 프로그램군은 3,709명으로 나타나 HPV 백신 접종으로 자궁경부암 발생 환자를 1,667명으로 줄일 수 있는 것으로 확인되었다. 이때 추가로 얻게 되는 전체 코호트의 기대수명은 1,648LYG, 질보정수명은 1,849QALYs로 산출되었다.

국가예방접종 도입 후 백신비용 인하율을 70%로 하고 HPV 백신 접종률을 86%로 가정했을 때 백신 접종 비용으로 약 909억원이 소요되는 것으로 나타났다. 예방접종 프로그램 도입 이후 CIN 발생 감소로 절감되는 비용은 약 174억원, 자궁경부암 발생 감소로 절감되는 의료비용은 약 139억원인 것으로 나타났다. 암 발생 감소 및 생존률 증가에 따라 검진비용은 약 2억 5천만원이 더 소요되는 것으로 추정되었다. 이를 종합하면 HPV 예방접종 프로그램 도입시 소요되는 총 비용은 1,670억원(할인율 5%적용)으로 도입 전 비용인 1,072억원에 비해 약 598억원이 추가로 소요되는 것으로 나타났다.
(3) 점증적 비용-효용비
비용-효용 분석 결과 HPV 예방접종 프로그램 도입시 1,849QALYs를 추가로 얻는 것에 대해 비용은 598억원이 더 소요되어 점증적 비용-효용비는 3,200만원/QALY로 산출되었다. 한국의 비용-효과성 기준을 2,000-3,000만원으로 볼 때(안정훈 등, 2010) 이는 비용-효과적이지 않은 대안으로 판단된다.
(4) 민감도 분석
민감도 분석 결과 접종률, 의료비용 자료원에 따른 의료비용 변화, 자궁경부암 재발환자에서의 사망률은 결과에 크게 영향을 주지 않았으나 백신가격 및 백신효과, 할인율 등은 비용-효과성 여부에 변화를 가져오는 것으로 확인되었다. HPV 국가예방접종 도입 후 현재의 백신비용 수준에서 50%까지 낮추거나 3회 모두 접종하여 접종효과를 극대화할 경우 HPV 예방접종이 비용-효과적이 되는 것으로 나타났다. 특히 HPV 16, 18번 감염 기여율이나 HPV 감염 후 CIN 1로 전이확률과 같은 CIN 및 자궁경부암 환자 수와 직접적으로 관련이 있는 요인들이 경제성 분석 결과에 민감하게 영향을 주었다. 또한 미래에 발생하는 비용과 편익에 대한 할인율도 분석 결과에 큰 영향을 주는 것으로 분석되었다. 이와 같이 경제성 분석을 수행함에 있어 여러 불확실성이 존재하고 있어 연구 결과 해석에 주의가 필요할 것이다. 또한 자궁경부암 신환자수가 증가하여 4,000명(임계값이 3,000만원일 때)~5,200명(임계값이 2,000만원일 때) 이상이 되거나 백신 1회 접종 비용이 9-11만원 수준이 되면 HPV 백신 프로그램이 비용-효과적이 되는 것으로 나타났다.
8. 결론 및 정책제안
경제성 분석 결과 HPV 예방접종 프로그램 도입시 1,849QALYs를 추가로 얻는 것에 대해 비용은 598억원이 더 소요되어 점증적 비용-효용비는 3,200만원/QALY로 산출되었다. 한국의 비용-효과성 판단 기준을 2,000-3,000만원으로 볼 때(안정훈 등, 2010) 이는 비용-효과적이지 않다고 볼 수 있다. 즉, 현재 한국의 상황에서 만 12세 여아에게 HPV 백신 예방접종 프로그램을 도입하는 것이 비용-효과적이지 않았다. 그러나 변수들의 변화에 따라 비용-효과성 여부가 민감하게 바뀌고 있으며 자궁경부암 관련 역학적 상황에 따라 HPV 예방접종 프로그램이 비용-효과적으로 전환될 가능성이 있어 이에 대한 고려가 필요할 것으로 판단된다.
WHO에서는 1) 자궁경부암 예방이 공공 보건 정책의 우선순위일 때, 2) 제도적으로 가능하고, 재원의 안정적 확보가 가능할 때, 3) 국가필수예방접종 도입이 비용-효과적일 때 HPV 예방접종 프로그램을 국가필수예방접종으로 도입하는 것을 고려할 수 있다고 밝히고 있다. 본 연구 결과를 바탕으로 제도 도입에 대한 우선순위 및 타당성을 검토하는 장이 마련되기를 기대한다.
1. Introduction
Governments and local organizations in America, Europe, and Australia are now beginning to fund vaccinations against human papilloma virus (HPV) program, following the WHO recommendation in 2009 that all women aged 9 to 13 years be vaccinated.
The HPV vaccines Gardasil (Merck&Co., Inc.) and Cervarix (GSK) have come to market in Korea, after receiving approval from the Korea Food & Drug Administration in June, 2007 and July, 2008, respectively. Korea has not yet introduced HPV vaccination into the national vaccination program, and there is ongoing disagreement on the subject of HPV vaccination among Korean academies.
Two previous cost-benefit studies in Korea showed that the cost of a national HPV vaccination program is high compared with the benefits, leading to the conclusion that the vaccination program is not cost-effective with the current price of HPV vaccines. Considerable uncertainty remains, however, because insufficient data caused the previous studies to rely on some questionable assumptions.
This study was performed with cost-utility analysis that is widely used to estimate the efficiency of HPV vaccination in the healthcare sector. This study also tried to elevate the completeness by incorporating data from the claims to the National Health Insurance (NHI) and from patient survey.
The purpose of this study is to generate useful information for rational healthcare-policy decision-making by conducting the economic evaluation of a national HPV vaccination program from a societal perspective.
2. HPV infection epidemiology
2.1. Methods
Literature review were conducted to examine sexual behavior, HPV vaccination rate, HPV infection rate, HPV type distribution in cervical intraepithelial neoplasia (CIN) and cervical cancer, disease statistics for CIN and cervical cancer, and survival rate of patients with cervical cancer. We preferred Korean epidemiologic data in our economic analysis. we used data from other countries to compare with the Korean data, or to substitute for it when necessary.
2.2. Results
Results from Kim et al. (2007), Shin et al. (2010), and the Sixth Health Behavior Online Survey on the Sexual Behavioral Pattern of Korean Adolescents showed that Korean women tend to have their first sex at a later age and tend to have sex at a lower frequency compared with American, European, and Australian women. Korean women have their first sex at 19 to 21 years of age, on average. A survey of women from 12 to 29 years of age in the six largest Korean cities (Seoul, Busan, Taegu, Inchon, Kwangju, and Taejon) found that 5% had their first sex under the age of 16.
While there are no official statistics on the HPV vaccination rate in Korea, one study (Bang et al., 2011) reported that 12% of a sample of 200 university students had received HPV vaccinations.
The prevalence of HPV infection among Korean women is estimated to be approximately 10-15%, with a higher prevalence typically among younger women (Shin et al., 2007). The infection rate among sexually experienced young women, 28.8%, was especially high compared with that among women in general, 15.2% (Shin et al., 2007). According to research on the HPV infection rate among sexually experienced women in general in 15 countries (Franceschi et al., 2006), African women had the highest rate of infection, 27%, and European women had the lowest rate of infection, 2.7-9.4%; the rate among Korean women was 14.8%. A systematic review of research on HPV types among Korean women found that HPV type 16 was the most common, with an infection rate of 6%, followed by HPV types 18 and 58 (Bae et al., 2008).
The rate of HPV infection was 63.2% among patients with premalignant cervical cancer (ASCUS or CIN 1), and that of high-risk HPV infection among patients with ASCUS or CIN 1 was 56.3%. Among patients with CIN 2, CIN 3, or cervical carcinoma in situ (CIS), the rate of HPV infection was 85.6%, and that of high-risk HPV infection was 83.7%. Among patients with invasive cervical cancer, the HPV infection rate was 88.3%, and that of high risk HPV infection was 84.6%. Data from East Asian countries display a similar trend, with Korea among the top five affected HPV types (Bae et al., 2008).
According to the Central Cancer Registry (2009), the number of new cervical cancer patients in Korea was 4,443 in 1999 and 3,733 in 2009, and the age- standardized incidence rate decreased from 18.6 persons in 1999 to 12.0 persons in 2009. Supposing that the average life span of Korean women is 84 years, the chance of a Korean woman having cervical cancer at some point in her life is 1.4%.
Survival data on Korean patients with cervical cancer are limited, so the survival rate is based mainly on the FIGO Annual Report, which contains Korean data. An analysis of 11,775 patients with cervical cancer found that: the 1-year survival rate was 89.8%; the 2-year survival rate was 80.7%; the 3-year survival rate was 75.5%; the 4-year survival rate was 72.3%; and the 5-year survival rate was 69.3%. The same data found that: the 1-year progression-free survival rate was 92.9%; the 2-year progression-free survival rate was 85.4%; the 3-year progression-free survival rate was 81.4%; the 4-year progression-free survival rate was 79.3%; and the 5-year progression-free survival rate was 77.7% (Quinn et al., 2006).
3. Clinical efficacy of HPV vaccines
3.1. Methods
We conducted a systematic review of the latest evidence on the clinical efficacy and safety of all types of HPV vaccines. We do not confine age, race, region, HPV infection and sexual experience. HPV types 16 and 18 related CIN2+, CIN3+ and persistent infection rate of 6/12months were reviewed as primary outcome.
We searched the domestic database such as KoreaMed, KISS, KMBASE, NDSL and overseas database such as Ovid-Medline, Ovid-EMBASE, CENTRAL (Cochrane Library). We conducted the risk of bias assessment on the selected literatures by first and second selection/exclusion process, using Cochrane’s Risk of Bias (RoB). Meta-analysis was performed categorizing the data by ITT (intent-to-treat), mITT (modified-intent-to-treat), and PP (per protocol) criteria.
3.2. Results
Nine clinical studies were selected among 5,661 literatures. It was estimated that the risk of bias in each study was almost low.
The relative risk (RR) of HPV types 16 and 18 relating CIN 2+ was 0.52 [95% CI 0.37-0.7] in ITT analysis, 0.09 [95% CI 0.04-0.22] in mITT analysis, 0.06 [95% CI 0.03-0.12] in PP analysis. Thus, all three analyses suggested that HPV vaccination significantly decreases the risk of HPV types 16 and 18 relating to CIN 2+ (Figure 1).
The RR in ITT analysis, representing the efficacy for all the vaccinated subjects regardless of their HPV infection, was 0.52, which leads us to conclude that the clinical risk of HPV types 16 and 18 related CIN 2+ is reduced by 48%. The clinical risk of developing HPV types 16 and 18 related CIN 2+ among the only HPV uninfected subjects vaccinated more than once is reduced by 91% based on the mITT analysis and that of developing HPV types 16 and 18 related CIN 2+ among the HPV uninfected subjects vaccinated entirely three times is reduced by 94% based on the PP analysis.
It was found that the persistent infection rate of HPV types 16 and 18 was decreased by 56% (RR = 0.44; 95% CI 0.34-0.57) according to the combined analysis of ITT and mITT analysis, and by 92% (RR = 0.08; 95% CI 0.04-0.15) according to the PP analysis. In the ITT and mITT combined analysis, the RR for the 12 month-persistent infection rate was 0.45 [95% CI 0.34-0.58] and in the PP analysis, the RR for the 12 month-persistent infection rate was 0.09 [95% CI 0.06-0.13] (Figure 2).
More studies are necessary for the conclusion that HPV vaccination provides cross protection against multiple virus types; because we currently do not have enough data on cross protection, and the data described here is only for the quadrivalent HPV vaccine. There is no clear evidence based on post-marketing analyses and safety data from clinical trials that HPV vaccines are not safe. Thus, we conclude that HPV vaccines currently appear to be safe. However, it is necessary to monitor the long-term outcomes of vaccinations and the results of post-marketing analyses.
4. The number of HPV relative patients and medical costs
4.1. Methods
NHI claims data provided by Health Insurance Review & Assessment Service (HIRA) was used to estimate the number of patients and the medical costs of HPV-related diseases. We included the patients with primary disease or first sub-ordinate disease related HPV infection, as classified by ICD-10 code.
We then conducted a more detailed analysis of patients with CIN 1, CIN 2/3, or cervical cancer, which were used in our economic model. We defined new patients as those who did not utilized medical services with CIN 1, CIN 2/3, or cervical cancer for the previous two years, and we used data from 2007 and 2008 as a wash-out period. After consulting a clinician, we defined the ICD-10 code N870 as being CIN1, the ICD-10 code N871 as being CIN2, and the ICD-10 code N872/D06 as being CIN3. We calculated the number of new patients with CIN separately, because there is no sub-code for N87, and the frequency of uncategorized ICD-10 codes, like N879, was around 50%.
Cervical cancer (C53) was analyzed with more specific health condition separating following up of stable condition and suffering a recurrent/persistent condition in next year after initial cervical cancer diagnosis. For this specific classification, patients suffering a recurrent/persistent condition are defined as patients who had got a surgery, chemotherapy, or radiation therapy at next year after initial cervical cancer diagnosis. Procedure codes for surgery, chemotherapy, and radiation therapy are also used along with claims code of HIRA with the consultation of clinicians.
4.2. Results
(1) Prevalence rate and medical costs of cervical cancer (C53)
The prevalence rate of cervical cancer was 90.8 per a population of 100,000 with a decreasing trend over recent years. It was most prevalent among women at the age of 40s to 60s, and the count of cervical cancer prevalent patients among women at 60s per 100,000 persons was 194.9. The average treatment cost per patient with cervical cancer was KRW 3,482,030 and it was found that the treatment cost per patient is increasing annually, while the number of the prevalent patients is decreasing annually.
(2) Incidence rate, recurrence rate, and medical cost of new patients
Depending on whether patients with uncategorized CIN were included, the number of patients for CIN 1 ranged from 25,946 (incidence rate 104.4 per 100,000 women) to 46,463 in 2009 (incidence rate 187.0 per 100,000 women), and it had a trend to increase from 35,018 (incidence rate 139.9 per 100,000 women) to 69,431 (incidence rate 277.4 per 100,000 women) in 2011.
Depending on the estimation methods, the estimated range of CIN 2/3 incidence rates per 100,000 women was 97.8-175.1 in 2009, decreasing to 88.1-174.8 in 2011.
The number of new patients with cervical cancer decreased from 5,593 (22.5 per 100,000 women) in 2009 to 4,958 (19.8 per 100,000 women) in 2011. The incidence rate of cervical cancer increased with age, peaking at 70 years of age (Figure 3). The rate of recurrence/persistence in next year after initial cervical cancer diagnosis was 5.44%. The rate of recurrence in the third year after diagnosis was 1.8%, and the rate of recurrence/persistence after an initial recurrence was 25.8%. Excluding the highest and lowest 3% of total patient costs as outlier, we estimated that in 2009 the average treatment cost per new patient with CIN 1 was KRW 209,979, while the average treatment cost per patient with CIN 2/3 was more than three times higher at KRW 679,291. The average treatment cost per patient in the first year following diagnosis of cervical cancer was KRW 6,570,000 while the average cost per follow-up patient without recurrence was KRW 1,000,000 and that per patient with recurrence was 16 times higher at KRW 16,220,000.
5. Cost analysis of cervical cancer relevant diseases
5.1. Methods
With the approval of the IRB of the NECA and other medical institutions, we performed a retrospective analysis of the 3-year medical costs for new patients diagnosed with CIN or cervical cancer in 2009 in six medical institutions.
5.2. Results
We identified 1,705 patients who had medical expenses recorded in 2009. We excluded 17 patients who had no medical expenses recorded during the first year following diagnosis. Among the 1,692 remaining patients, 288 had CIN 1, 675 had CIN 2/3, and 729 had cervical cancer; 655 (89.8%) of the patients with cervical cancer had a first-time diagnosis and not a recurrence.
The average treatment amount per patient in the first year following CIN 1 diagnosis was about KRW 560,000 and the average medical costs was about KRW 850,000. The average amount of non-health insurance benefit was KRW 290,000 and the average out-of-pocket payment was about KRW 510,000. The percentage of out-of-pocket payment was 72.5% on average per patient and the percentage of non-health insurance benefit was 37.5% on average per patient.
The average treatment amount per patient of a first year CIN 2/3 diagnosed patients was about KRW 820,000 and the average medical costs was about KRW 1,200,000. The average amount of non-health insurance benefit was about KRW 370,000 and the average out-of-pocket payment was about KRW 630,000. The percentage of out-of-pocket payment was 58.5% on average per patient and that of non-health insurance benefit was 35% on average per patient.
The average medical cost per patient following a non-recurrent cervical cancer diagnosis was KRW 18,400,000 in the first year, KRW 1,700,000 in the second year, KRW 1,200,000 in the third year. Thus, the annual medical costs gradually decreased among non-recurrence patients. The frequencies of hospital stays and outpatient visits and the average length of hospital stays also decreased from year to year.
The average medical cost per patient following a recurrent cervical cancer was KRW 30,000,000 in the first year, KRW 6,560,000 in the second year and KRW 1,650,000 in the third year.
6. Survey on cervical cancer patients
6.1. Methods
With the approval of the IRB of the NECA and other medical institutions, we conducted a survey from Oct. 2012 to Dec. The quality of life and medical expenses of 452 patients with CIN or cervical cancer who were treated in department of gynecology at one of six medical institutions in Seoul and the surrounding metropolitan areas. We measured the patients’ quality of life using the EQ-5D tool. The questionnaires included demographic information, quality of life, unofficial medical costs, time costs, and caregiver costs.
6.2. Results
The utility weight was highest among CIN 1 patients at a valuation of 0.933, followed by those among patients with CIN 2/3, at a valuation of 0.933, among patients with cervical cancer, at a valuation of 0.874, and among patients with recurrent cervical cancer, at a valuation of 0.784. Thus, the severity of the disease was in inverse proportion to the quality of life.
The unofficial medical costs for the first year were highest among patients with cervical cancer (including recurrent cervical cancer), and tended to decrease as the treatment period became longer. The more severe the cervical cancer was and the shorter the treatment period was (less than one year after diagnosis), the more caregiving days and caregiving hours were required. The average time required to visit a hospital was 1.7 hours for patients with CIN 1, 4.2 hours for patients with CIN 2/3, 4.7 hours for patients with first-diagnosis cervical cancer, and 5.6 hours for patients with recurrent cervical cancer.
7. Economic evaluation of HPV vaccination
7.1. Methods
(1) Model overview
Economic evaluation was conducted to compare a national HPV vaccination program that vaccinates 12-year-old girls with cervical cancer screening and the current system that provides only cervical cancer screening and no HPV vaccination program. We analyzed the effect of the vaccination program on the rates of HPV infection and cervical cancer and conducted a cost-utility analysis applying quality adjusted life years (QALYs), which take into account the extension of life span and the quality of life. We took a societal perspective, considering patient time costs, caregiver costs, and transportation costs, in addition to medical costs. We used a Markov model with a 1-year cycle and lifetime analysis period. The Markov states were: well, HPV infection, CIN 1, CIN 2/3, cervical cancer (initial cancer), follow-up cervical cancer, recurrent/persistent cancer, follow-up recurrent/persistent cancer, and death. We limited the HPV infections caused by HPV types 16 and 18 (Figure 4). We conducted the analysis with the assumptions that vaccination does not affect the current cervical cancer-screening rate and that the effect of the vaccine lasts for a lifetime. We did not consider any herd-immunity effects or cross-protection effects. We estimated the rate of HPV infection by calibrating using the age-specific HPV prevalence, frequency of sexual activity, and HPV infection-regression rate. The annual rate of cervical cancer screenings among Korean women was used as the proportion of CIN treatment and management. Claims data from the NHI was analyzed to estimate the transition probabilities from CIN 1 to CIN 2/3 and from CIN 2/3 to cervical cancer. The FIGO annual report (Quinn et al., 2006) and a review of previous research were used to estimate death rate for each health state.
(2) Costs
We estimated the annual cost of vaccines to be KRW 343,144, using equal proportion of the two vaccines on the Korean market, Gardasil and Cervarix, and assuming that the costs would be 70% less than the current market price once a national vaccination program is implemented. We calculated the cost of screening for cervical cancer using the age-specific national cancer-screening rate and the private cancer-screening rate. We computed the annual medical costs per patient diagnosed with CIN or cervical cancer from the NHI claims data, and we estimated the unofficial medical costs based on the results of the patient surveys. We estimated the transportation costs, time costs, and caregiver costs from the NHI claims data, the patient surveys, and the unit costs reported in previous research.
(3) Effects
We used the persistent infection rates of HPV types 16 and 18 obtained from our meta-analysis of the systematic review. We applied the result of the mITT combined analysis, RR 0.45 [CI 0.34-0.58], to estimate the effect of subjects being vaccinated more than once.
(4) Sensitivity analysis Sensitivity analysis was conducted to reflect the uncertainty among the variables for the vaccine cost, vaccination rate, vaccine efficacy, discount rate, medical costs, and death rate among patients with recurrent cervical cancer.
7.2. Results
(1) Total effectiveness
Our model showed that for a cohort of 12-year-old girls (n=300,405), a national HPV vaccination and cervical cancer screening program would result in 2,042 patients developing cervical cancer, whereas the current screening program would result in 3,709 patients developing cervical cancer. Thus, the number of cases of cervical cancer would be reduced by about 45%. From this, we estimated that the cohort would get a total of 1,648 life years gained (LYG) and an additional 1,849 QALYs.
(2) Total costs
We estimated that a national HPV vaccination program would cost KRW 90.9 billion to implement, assuming the vaccine cost is reduced to 70% of the current price and 86% of eligible women receive vaccinations, and reduce the costs associated with CIN and cervical cancer by KRW 17.4 billion and KRW 13.9 billion, respectively. We estimate that the total cost of a national HPV vaccination program would be KRW 167 billion (with a discount rate of 5%) compared with the current screening program cost of KRW 107.2 billion, thus requiring an additional KRW 59.8 billion.
(3) Incremental cost-utility Ratio According to cost-utility analysis, the KRW 59.8 billion implementation of a national HPV vaccination program would generate an additional 1,849 QALY, providing an incremental cost-utility ratio of KRW 32 million/QALY. Considering the threshold of Korean cost-effectiveness, which is KRW 20 million to 30 million (Ahn et al., 2010), we conclude that the vaccination program is not cost-effective.
(4) Sensitivity analysis
Sensitivity analysis showed that the vaccination rate, medical costs, and death rate among the patients with recurrent cervical cancer did not affect the result significantly, whereas the vaccine cost, the vaccine efficacy, and the discount rate strongly impacted on the cost-effectiveness of the vaccination program. A national HPV vaccination program would be cost-effective if the vaccine cost drops by 50% or when individuals are fully vaccinated three times after the program is implemented, thus maximizing the effectiveness of the vaccine. The number of patients with cervical cancer and a discount rate for future costs and health outcomes had especially strong effects on the results of the economic evaluation.
8. Conclusion and Policy suggestions Although the HPV vaccination program for 12-year-old girls was not cost-effective under the current conditions in Korea, the cost-effectiveness is very sensitive to changes in certain variables. The WHO recommends introducing national HPV vaccination programs when: 1) the prevention of cervical cancer is a priority for public health policy, 2) sustainable financing is available, and 3) the introduction of a national vaccination program is cost-effective. This study will help to prioritize and validate plans to introduce national HPV vaccination programs.
