▢ 평가배경 본 평가의 대상기술인 ‘폐쇄 마이봄선 가열 치료법’은 마이봄선의 기능저하나 마이봄선과 라크리말선의 복합이상으로 인한 안구 건조증 환자에서 마이봄선 안쪽에서 온열하여 막힌 마이봄샘 관을 뚫어주어 안구건조증의 증상완화 및 치료하기 위한 기술이다. 해당기술은 2013년 신의료기술평가 결과 ‘폐쇄 마이봄선 가열 진동 치료법(Thermal Pulsation Treatment for Obstructive Meibomian Gland Dysfunction)’ 이란 기술명으로 고시되었으나(보건복지부고시 제2014-20호, 2014.2.6.) 2016년 12월에 개최된 건강보험심사평가원의 의료행위전문평가위원회에서 최종 고시명이 ‘폐쇄 마이봄선 가열 치료법’으로 변경되었다(보건복지부고시 제2017-37호(2017.3.1.). 보건복지부는 비급여 항목 485개 기술을 모두 급여항목으로 전환하기 위한 절차를 진행 중에 있으며, 동 기술은 2020년에 전환 예정인 항목 중 신의료기술평가를 받은 기술에 해당하여 이번 의료기술재평가 사업(과제번호: NR19-001, 연구책임자: 최인순)의 일환으로 근거 업데이트를 수행하게 되었다.
▢ 위원회 운영 폐쇄 마이봄선 가열 치료법 소위원회의 위원은 총 5인의 관련분야 전문가로 구성하였고 총4회의 소위원회를 통해 동 기술의 안전성 및 유효성을 평가하였다. 소위원회는 전문분야별로 근거기반의학(호흡기내과) 1인, 안과 4인 총5인의 전문의로 구성하였다. 위원들은 연구계획서 작성부터 PICO 형식에 의한 검색어 선정, 선택 및 배제기준 등 모든 부분에 참여하여 체계적 문헌고찰 방법으로 평가하였으며, 객관적인 전문가 자문을 함께 수행하였다. 또한 4차 소위원회에서 최종 결론을 확정하였으며, 이후 보완된 최종보고서 서면자문을 통해 최종 확정하였다.
▢ 평가목적 및 방법 폐쇄 마이봄선 가열 치료법은 마이봄선의 기능저하나 마이봄선과 라크리말선의 복합 이상으로 인한 안구 건조증 환자에서 마이봄선 안쪽에서 온열과 진동을 주어 막힌 마이봄선 관을 뚫어주어 안구건조증의 증상완화 및 치료를 목적으로 시행하는 시술로 동 시술의 안전성과 유효성을 아래와 같이 체계적 문헌고찰을 통해 검토하였다. 폐쇄 마이봄선 가열 치료법의 문헌검색전략은 안검염 또는 마이봄선염을 동반한 안구 건조증 환자를 대상으로 폐쇄 마이봄선 가열 치료법을 수행하고 관습적인 치료법과 비교하여 동 동 시술의 시술 후 합병증 또는 부작용과 마이봄선 기능 호전, 눈물 파괴 시간, 증상완화, 안구표면염색을 주요 의료결과로 선정하였다. 폐쇄 마이봄선 가열 치료법에 대한 검색은 KoreaMed를 포함한 5개 국내 데이터베이스와 국외의 Ovid-MEDLINE, Ovid-EMBASE, Cochrane Library 데이터베이스를 이용하였다. {(Dry Eye Syndromes.mp. OR exp Dry Eye Syndromes/) OR (Meibomian Glands.mp.) AND (Thermodynamics.mp. or exp Thermodynamics/ AND Thermal Pulsation System treatment.mp.)}를 통합한 검색전략을 통해 총 200건을 검색하였고, 동물실험이나 전 임상연구, 원저가 아닌 연구, 한국어와 영어로 출판되지 않은 연구, 증례연구 또는 증례보고, 적절한 의료결과가 하나 이상 보고되지 않은 연구의 경우는 배제하였다. 중복검색된 문헌을 제거하고 총 142편(국외 142편, 국내 0편)이 확인되었다. 원문 검토후 129편을 배제한 후 소위원회에서 확정하여 최종 13편을 선정하였다. 연구유형별로는 무작위임상시험 4편, 비무작위임상시험 2편, 전후연구 7편이었다. 문헌검색부터 선택기준 적용 및 자료추출까지 각 단계는 모두 소위원회와 아울러 2명의 평가자가 각 과정을 독립적으로 수행하였다. 문헌의 비뚤림 위험 평가는 RCT의 경우 Cochrane RoB, NRS 및 전후연구의 경우 RoBANS II 도구를 이용하였다. 그리고 근거수준은 GRADE를 이용하였다.
▢ 평가 결과 <안전성> 폐쇄 마이봄선 가열 치료법의 안전성은 3편의 RCT연구(Blackie et al 2018, Blackie et al 2016, Lane et al 2012)와 1편의 전후연구(Friedland et al 2011)로, 시술관련 합병증과 최대교정시력, 불편감/통증, 안구착색, 안압으로 평가하였다. 3편의 RCT연구에서 중증 합병증은 보고되지 않았다. 안전성을 보고한 문헌에서 Blackie 등(2016) 연구에서는 시술관련 경증합병증의 경우 19건이 보고되었다. 치료와 관련된 시술을 받은 치료군과 교차시험에 해당하는 연구대상자중 5.1%, 비교시술군인 온열요법에서는 7.1%에서 발생하였다. 그중 가장 흔한 기기관련 합병증은 치료군에서 불편감/통증(1.5%), 비교군에서 피부염(1.5%)이었다. Lane 등(2012) 에서는 안검 통증, 결막의 혈관 감염, 작열감과 같은 경증 합병증은 4주안에 후유증이나 약물치료 필요 없이 호전되었다. 최대교정시력이 2줄 이상 증가되어 연구시작점보다 의미 있게 악화된 안구의 비율은 치료군이(21.5%) 비교 군(42.9%)보다 낮았으나 통계적으로 유의하지는 않았다(p=.68). 치료 중 불편감/통증은 동 시술이 온열요법보다 높았으나, 시술의 특성상 눈이 노출되면서 자극이 되기 때문에 발생 하는 문제로 결막이나 각막에 상처가 발생한 사례는 보고되지 않았다. 안구착색은 시술 후 또는 하루만에 회복되었고, 안압은 두 군이 유사하게 감소되었다. 본 평가에서는 GRADE를 이용한 평가가 추가되었다. 결과는 안전성을 정량적으로 보고한 1편(Lane et al 2012)에서 근거수준은 Moderate로 평가되었다. 소위원회에서는 동 시술은 심각한 시술관련 합병증이 보고되지 않았고, 부작용이 발생하더라도 치료 후 또 다른 시술을 요하지 않고 완화되어 안전한 시술이라는 의견이었다. <유효성> 폐쇄 마이봄선 가열 치료법의 유효성은 4편의 RCT연구(Blackie et al 2016, Blackie et al 2018, Finis et al 2014, Lane et al 2012)에 대하여 마이봄선 기능호전정도, 눈물 파괴시간, 안구건조증 증상 완화정도, 안구형광염색점수로 평가한 결과에서 온열요법과 비교한 평균 3개월 추적관찰 결과는 아래와 같다. 마이봄선 기능 호전(MGS) 폐쇄 마이봄선 가열 치료법이 온열요법보다 유의하게 마이봄선 기능이 호전되었다(7.17, 95%CI [5.57, 8.78]). 눈물파괴시간(TBUT) 폐쇄 마이봄선 가열 치료법이 온열요법보다 유의하게 눈물 파괴 시간이 길었다(1.44, 95%CI [0.47, 2.41]). 증상호전(OSDI, SPEED) 폐쇄 마이봄선 가열 치료법이 온열요법보다 유의하게 증상이 더 향상(SPEED: -2.84 95%CI [-4.47, -1.20]; OSDI: -7.33, 95%CI [-10.22 , –4.44])되었다. 안구표면염색(OSS) 안구표면염색점수는 온열요법이 치료군에 비해 증가(OSS: 0.89, 95%CI [-1.84, 3.63]) 하였으나 유의하지 않았다. GRADE 결과에서 근거수준은 마이봄선기능호전(MGS)과 증상호전(OSDI)은 High, 증상호전(SPEED)과, 눈물파괴시간(TBUT)은 Moderate, 안구표면염색(OSS)은 Very low로 평가되었다.
▢ 결론 폐쇄 마이봄선 가열 치료법 소위원회에서는 이러한 문헌적 근거를 토대로 다음과 같이 검토결과를 제시하였다. 이러한 문헌적 근거를 토대로 폐쇄 마이봄선 가열 치료법은 장기간 효과를 볼 수 있는 비교 연구근거가 다소 부족하지만, 무작위배정 비교임상시험을 근거로 하여 안검염 또는 마이봄선염을 동반한 안구건조증 환자에서 마이봄선기능과 증상을 호전시켜주는 안전성 및 유효성이 있는 시술이라고 평가하였다. 의료기술재평가위원회는 “기술항목명(폐쇄 마이봄선 가열 치료법의 안전성 및 유효성 평가)”에 대해 소위원회 검토결과가 타당하다고 심의하였다(2019.09.20.).
Thermal treatment for obstructive meibomian gland, which is the
technology being assessed, is a procedure used to treat and alleviate symptoms
of dry eye syndrome by unblocking the meibomian gland duct by heating from
inside the meibomian gland in patients with dry eye syndrome caused by meibomian
gland dysfunction or combination of abnormalities involving the meibomian gland
and lacrimal gland.
This technology was announced by the name “thermal
pulsation treatment for obstructive meibomian gland dysfunction” in the new
health technology assessment in 2013 (Ministry of Health
and Welfare notification 2014-20, February 6, 2014), but the name was
subsequently changed to “thermal treatment for obstructive meibomian gland” by
the medical practice professional assessment committee meeting held by the
Korean Health Insurance Review and Assessment Service in December 2016 (Ministry
of Health and Welfare notification 2017-37, March 1, 2017).
The Ministry of Health and Welfare is
in the process of converting 485 technologies that are non-covered items into
covered items. This technology has already been
assessed as a new health technology among the items to be decided by 2020.
Accordingly, evidence update work was performed as a part of the latest health
technology reassessment project (Project number: NR19-001, Principal
investigator: In-Soon Choi).
The subcommittee on thermal treatment for obstructive meibomian
gland consisted of five experts from relevant fields [four ophthalmologists and
one evidence-based medical expert (pulmonology)] and a total of four
subcommittee sessions were held to assess the safety and effectiveness of this
technology.
1) The members participated in all
processes from drafting the research protocol, selection of search terms based
on PICO format, to establishment and application of selection and exclusion
criteria to carry out the assessment by a systematic literature review, while
also acting as objective expert advisors. Moreover, the final conclusions were
derived during the fourth subcommittee session and was finalized through
written advice on the subsequently supplemented final report.
▢ Assessment objectives and methods
Thermal
treatment for obstructive meibomian gland is
a procedure used to treat and alleviate symptoms of dry eye syndrome by
unblocking the meibomian gland duct by applying heat and pulsation from inside
the meibomian gland in patients with dry eye syndrome caused by meibomian gland
dysfunction or combination of abnormalities involving the meibomian gland and
lacrimal gland. The safety
and effectiveness of this technology were assessed by a systematic literature
review as described below.
As the search strategy for articles about thermal
treatment for obstructive meibomian gland, the major health outcomes selected
consisted of postoperative complications or adverse events, improvement in
meibomian gland function, tear break-up time (TBUT), symptom alleviation, and ocular
surface staining (OSS) of this procedure applied to patients with dry eye
syndrome comorbid with blepharitis or meibomianitis, as compared to conventional
treatment methods.
The search for thermal treatment for obstructive meibomian
gland used five Korean databases, including KoreaMed, and foreign databases,
including Ovid-MEDLINE, Ovid-EMBASE, and Cochrane Library. Based on a search
strategy that combined {(Dry Eye Syndromes.mp. OR exp Dry Eye Syndromes/) OR
(Meibomian Glands.mp.) AND (Thermodynamics.mp. or exp Thermodynamics/ AND
Thermal Pulsation System treatment.mp.)}, a total of 200 articles were
searched, while the following types of studies were excluded: animal studies,
pre-clinical trials, non-original studies, studies not published in Korean or
English, case studies, case reports, and studies that did not report at least
one applicable health outcome. After excluding duplicately searched articles, a
total of 142 articles (0 domestic and 142 foreign articles) were identified.
After excluding 129 articles based on a full text review, 13 articles were
ultimately selected with confirmation from the subcommittee. When classified by
study types, there were four randomized clinical trials (RCTs), two non-RCTs,
and seven pre-post studies.
All steps, from article search,
application of the selection criteria, to data extraction, were performed
independently by the subcommittee and two assessors. Risk of bias was assessed using Cochrane’s risk of bias
for RCTs and RoBANS II for non-RCTs and
pre-post studies. The level of evidence was assessed using the GRADE method.
<Safety>
The safety of thermal treatment for obstructive meibomian
gland was reported in three RCTs (Blackie et al., 2018; Blackie et al., 2016; and
Lane et al., 2012) and one pre-post study (Friedland et al. 2011), which was
assessed by procedure-related complication, best-corrected visual acuity,
discomfort/pain, ocular staining, and intraocular pressure (IOP).
Three RCT reported no severe complications. Among the studies
that reported on the safety, the study by Blackie et al. (2016) reported 19
cases of mild procedure-related complications. The incidence was 5.1% among
study subjects corresponding to cross test with the intervention group that
received the treatment-related procedure and 7.1% among the control group which
received heat therapy. The most common
equipment-related complications were discomfort/pain (1.5%) in the intervention
group and dermatitis (1.5%) in the control group. The study by Lane et al. (2012)
reported that mild complications, such as palpebral pain, vascular infection of
the conjunctiva, and burning sensation, improved within four weeks without any
drug therapy or sequelae. The percentage of eyes with significant deterioration
in best-corrected visual acuity with increase of ≥ 2 lines, as compared to the
baseline, was 21.5% in the intervention group and 42.9% in the control group,
but the difference was not statistically significant (p=.68). Incidence of
discomfort/pain during the treatment was higher in this procedure than heat
therapy, but such problem occurred due to the characteristics of the procedure
that causes the eye to be irritated from being exposed. However, there were no
reported cases of injury to the conjunctiva or cornea. Ocular staining
recovered after the procedure or within one day, while both groups showed
similar level of decrease in IOP. In this assessment, assessment using GRADE
was added. The results showed that the level of evidence was moderate in one
article (Lane et al., 2012) that quantitatively reported on safety. The
subcommittee opined that thermal treatment for obstructive meibomian gland is a
safe procedure since no serious procedure-related complications were reported
and even the adverse events that occurred dissipated after the treatment
without any other interventions.
<Effectiveness>
The effectiveness of thermal treatment for obstructive meibomian
gland was reported in four RCTs (Blackie et al., 2016; Blackie et al., 2018;
Finis et al., 2014; and Lane et al., 2012), which was assessed by degree of
improvement in meibomian gland function, TBUT, degree of alleviation in dry eye
syndrome symptoms, and ocular fluorescence staining score. The results of
comparison with heat therapy during follow-up for an average of three months
were as described below.
Improvement in meibomian gland function
(MGS)
Thermal treatment for obstructive meibomian gland showed
significantly greater improvement in meibomian gland function than heat therapy
(7.17, 95%CI [5.57, 8.78]).
Tear break-up time (TBUT)
Thermal treatment for obstructive meibomian gland showed
significantly longer TBUT than heat therapy (1.44, 95%CI [0.47, 2.41]).
Symptom improvement (OSDI, SPEED)
Thermal treatment for obstructive meibomian gland showed
significantly greater symptom improvement than heat therapy (SPEED: -2.84 95%CI
[-4.47, -1.20]; OSDI: -7.33, 95%CI [-10.22 , –4.44]).
Ocular surface staining (OSS)
OSS score was higher with heat therapy than in the intervention
group (OSS: 0.89, 95%CI [-1.84, 3.63]), but the difference was not significant.
GRADE
results showed that the level of evidence was high for improvement in meibomian
gland function (MGS) and symptom improvement (OSDI); moderate for symptom improvement
(SPEED) and TBUT; and very low for OSS.
Based on such literary evidence, the subcommittee on thermal
treatment for obstructive meibomian gland presented the following review
results.
Based on such literary evidence, it was determined that
although evidence in comparative studies for long-term effects of thermal
treatment for obstructive meibomian gland is insufficient, thermal treatment
for obstructive meibomian gland was assessed to be a safe and effective
procedure that could improve meibomian gland functions and symptoms in patients
with dry eye syndrome comorbid with blepharitis or meibomianitis based on
evidence from RCTs.
The
Health Technology Reassessment Committee reviewed and determined that the
findings of the subcommittee on “safety and effectiveness assessment of thermal
treatment for obstructive meibomian gland (technology name)” are valid
(September 20, 2019).
