▢ 서 론
Ⅰ. 연구배경
보건의료계의 경우 공공보건의료가 소유·주체 중심에서 공공의 이익 실현의 개념으로 전환되고 있는 반면, 공공 분야에서 구강보건의료, 필수구강(보건)의료의 개념과 역할에 대한 정부와 치과계의 심도있는 논의와 연구가 부족한 상황이다. 구강보건의료 분야에서 공공의료의 개념과 역할을 정립하고, 이에 맞는 공공구강보건의료정책을 추진할 필요가 있다.
2022년 기준, 국공립의 비중은 치과병원이 3.34%, 치과의원원이 0.07%로 우리나라 구강의료전달체계는 민간 분야 중심으로 제공되고 있다. 구강질환에 대한 기본 치과진료인 예방, 치주, 보존치료에 대한 낮은 수가로 인해 민간분야에서는 적극적인 구강질환 관리가 이뤄지고 있지 않으며, 보철 및 비급여 진료가 높은 비중을 차지하고 있다. 보건소에서 제공하는 구강보건의료서비스의 종류는 제한적이며, 일부 대상자에 한정되며, 전담 치과의사, 치과위생사 인력의 지역간 편차가 크며, 부족한 실정이다. 보건복지부, 질병관리청과 같은 국가의 구강보건정책을 개발, 수행하는 국가기관의 구강보건의료 전문인력의 채용 또한 타 보건의료전문 인력에 비해 전무하거나 매우 적은 실정이다.
아동구강건강실태조사와 국민건강영양조사 구강조사 부문은 우리나라 국민의 구강건강을 모니터링하는 대표적인 공공 구강보건의료 조사체계이지만, 단면조사, 부족한 예산, 짧은 조사 기간, 전문 조사팀의 부재, 치과의사 조사자 구인의 어려움 등으로 인해 양질의 구강건강 지표가 산출이 점점 어려워지고 있는 실정이다. 효과적인 구강보건정책의 개발과 평가를 위해 현 구강보건조사체계에 대한 검토와 개선이 필요하다.
Ⅱ. 연구목표
공공분야 구강보건인력 및 구강진료기관의 수급과 운영 현황, 예방 및 구강질환관리 실태, 그리고 구강보건정책관련 법적, 행정적 현행제도를 진단하여 공공구강보건의료의 개념 정립 및 범위를 규정함으로써 미래사회에 대응할 수 있는 공공구강보건정책 개선방안을 마련하고자 한다. 따라서 본 연구는 공공구강보건 개선을 위한 우선순위를 파악하고자 탐색적 조사방법을 채택하여 문헌조사, 전문가 의견조사(자문회의, 공청회, FGI, 설문조사), 사례조사를 수행하였다.
▢ 주요 연구내용 및 결과
Ⅰ. 공공보건의료 및 구강보건의료분야 정책 동향
2000년에 제정되고, 2012년에 전면 개정된 공공보건의료에 관한 법률(공공의료법)에 따르면 공공보건의료란 국가, 지방자치단체 및 보건의료기관이 지역·계층·분야에 관계없이 국민의 보편적인 의료 이용을 보장하고 건강을 보호·증진하는 모든 활동을 의미한다. 2018년 발표된 공공보건의료 발전 종합대책에 따르면, 지금까지 공공보건의료는 여전히 취약지, 취약계층, 시장 실패 등 잔여적 접근 형태로 운영되는 실정으로, 미충족된 분야를 보완하는 수준의 기능만으로는 현재의 의료공급체계 문제와 급증하는 의료수요에 대응이 곤란하다고 진단하고, 전 국민의 필수의료 보장과 효과적 전달을 위해 모든 국민을 대상으로 생명·건강, 삶의 질과 직결되는 필수의료에 대해서 선제적·기본적인 역할을 수행하는 것으로 기능을 확대하기로 하고, 공공보건의료발전위원회 논의를 거쳐 ‘필수의료’의 범위를 설정한 바 있다. 2019년에는 필수의료분야 건강격차 완화를 위해 지역의료 자원을 육성하고 지역의료 협력 활성화를 목표로 하는 지역의료 강화 대책이 발표되었다. 2021년 공표된 제2차 공공보건의료 기본계획에 따르면, 민간의료기관이 90%에 이르는 보건의료 환경에서 소유·주체·보완 중심에서 공공의 이익 실현이라는 기능·역할·보편 중심으로 공공보건의료의 개념을 전환하고, 공공보건의료전달체계 구축 계획을 수립하였다.
이러한 정부의 공공보건의료체계 구축을 위한 논의 과정 속에서 공공구강보건의료에 대한 논의는 사실상 전무하였다. 보건의료전달체계 하에서 치과는 상급종합병원에서는 필수진료과목으로 포함되어있지만, 종합병원 치과는 선택 사항이며, 응급의료기관에서 치과의사 배치는 의무가 아니다. 치과대학/치의학전문대학원이 있는 전국 11개 대학교의 치과병원은 국내 구강보건의료 전달체계 하에서 실질적으로 상급종합병원에 준하는 역할을 하고 있으나, 법·제도적으로 민간 치과병의원과 구분되어 있지 않았다. 의과의 경우, 환자가 상급종합병원 진료를 받으려면 의원, 병원, 종합병원의 진료 의뢰가 필수이지만, 대학교 치과병원, 상급종합병원 치과는 환자는 아무런 제약없이 진료를 이용할 수 있어, 구강보건의료전달체계가 확립되어 있지 않았다.
Ⅱ. 공공구강보건의료 실태
공공구강보건의료 서비스를 수행할 구강보건진료인력에는 치과의사, 치과 공중보건의사, 치과의사전문의, 치과위생사, 치과기공사, 간호사, 간호조무사가 있다. 우리나라 인구 천명당 활동치과의사 수는 OECD 자료 2021년 기준 0.5명으로 OECD 국가 대비 상대적으로 적으며, 대부분이 민간부문에 종사하며, 공공부문에 종사자는 매우 적은 현실이다.
구강보건법(2000년 제정)은 구강보건사업을 효율적으로 추진함으로써 국민의 구강질환을 예방하고 구강건강을 증진함을 목적으로 하고 있으며, 이 법에 따라 보건소에는 구강질환 예방 및 진료를 위해 구강보건실 또는 구강보건센터를 설치·운영해야 하며, 장애인의 구강보건 및 구강건강증진에 관한 업무를 수행하기 위하여 중앙장애인구강진료센터를 설치·운영해야 하며, 권역장애인구강진료센터 및 지역장애인구강진료센터를 설치·운영할 수 있다. 2023년 현재 1개의 중앙 센터와 14개의 권역 센터가 운영 중이다. 전국 256개 보건소 중, 구강보건실은 87%, 구강보건센터는 23% 설치되어 있다. 하지만, 이들 기관의 치과의사, 치과위생사 전문인력과 예산 부족, 리더쉽과 역량 부족으로 인해 기관별 운영 실적의 차이가 발생하고 있다.
서울대학교치과병원 설치법(2004년 제정), 국립대학치과병원 설치법(2008년 제정)에 따라 치의학과가 설치된 국립대학과 서울대학교에 설치된 치과병원(국립대학 5곳, 서울대학교 2곳)은 공공보건의료에 관한 법률 제2조제2호에 따른 공공보건의료사업 수행의 책무가 있으며, 공공보건의료계획의 수립 및 시행결과에 대한 사항을 보고해야 한다. 이들 기관은 주로 취약계층을 대상으로 진료비 감면, 이동진료사업 등을 전개하는 것으로 파악되었으며, 일부 치과병원은 공공보건의료계획에 관한 내용이 공개되지 않아, 확인이 불가하였다.
국가가 법령에 근거해 운영하는 지역거점 공공보건의료기관(지역의료원 및 적십자병원) 38곳 중 치과진료서비스가 제공되는 기관은 28개소이며, 기관에 따라 인력과 시설의 차이가 존재하였다.
구강보건의료의 공공성 확대를 위해 치과진료 분야 건강보험 보장성이 확대되었으며, 아동에 대해 정기구강검진, 치면열구전색술 및 광중합형 복합레진 충전 급여 확대, 아동치과주치의 건강보험 시범사업이 추진 중이며, 성인에 대해 정기구강검진, 스케일링 급여화가 이뤄졌으며, 노인에 대해 노인틀니, 치과임플란트 급여화, 장기요양보험법에 따른 재가급여, 시설급여 구강관리서비스가 제공되고 있다. 장애인에 대해 장애인구강진료센터 설치, 치과적 중증장애인 치과 비급여진료비 지원, 장애인치과주치의 시범사업이 추진 중이다.

▢ 결론 및 정책적 제언
공공구강보건의료의 개념, 역할, 범위를 규정하고 필수구강(보건)의료 영역을 새로이 마련하였으며 이에 대한 관련 전문분야의 의견수렴을 통해 공통된 방향과 세부(안)을 마련하였다. 이를 통해 정의된 필수구강(보건)의료의 현황을 파악하기 위해 각 세부항목별 매핑분석을 실시하여 미래사회에 대응할 수 있는 공공구강보건정책 개선방안을 도출하고자 하였다. 필수구강(보건)의료 영역의 상당수가 비급여 의료에 포함되어있는 상황에서 모든 국민에게 필수적이고 보편적으로 제공하기 위한 보편성의 관점을 최우선시하되 장애인 및 취약계층 구강보건문제 등을 해결하기 위한 기본적인 1차 치과의료 제공과 구강건강 형평성의 관점을 같이 고려한 구강보건의료정책 방향을 제시하였다. 또한 예방중심의 필수구강(보건)의료 서비스의 확대를 위해서는 치과의료서비스체계의 다수를 차지하는 민간의료기관을 활용할 수 있는 방안을 적극적으로 모색해야 할 필요성을 강조하였다. 향후 공공보건의료기관과 민간의료기관의 협진체계를 구축하고 건강보장의 공공성을 강화하기 위한 연구가 이루어져야 한다.
의과분야 중심의 현 의료전달체계에 맞춰진 치과의료분야는, 대학교의 치과대학이 있는 치과병원은 의료전달체계 내 3차(상급종합병원급)가 아닌 2차(병원, 종합병원급) 의료기관으로 분류되는 것은 고난이도의 구강진료와 임상교육, 치의학 분야의 연구 환경 등을 고려했을 때 괴리가 존재한다. 또한 국립치과병원설치법에는 공공보건의료사업을 하도록 명시되어 있으나 공공분야 사업운영은 미약한 상황이다. 이에 대한 공공구강보건의료 및 필수구강(보건)의료의 제공에 대한 책무성을 강조하였고 향후 보다 적극적인 실행방안을 구축해야 할 필요성이 있다.
보건소 및 지역거점공공의료기관에는 구강보건의료인력의 비중이 매우 낮은 상황이며, 광역단위(시도) 구강보건사업 담당자는 전담인력이 아닌 타 여러 사업과 병행하고, 비전공자가 담당하는 경우가 자주 발생하여 구강보건사업의 원활한 운영이 어려운 상황이다. 공공구강보건의료를 강화하기 위해서는 지속적인 인력 확보와 안정적인 시설 운영 지원이 지속적으로 이루어져야 할 것이다.
위 내용을 정리하자면, 보편적인 구강의료이용 보장을 위해 1차 의료기관 중심 필수구강(보건)의료 서비스 전달체계 구축이 필요할 것이며, 필수구강(보건)의료 서비스 제공을 위한 사업 매뉴얼 개발이 이루어져야 할 것이다. 또한 필수구강(보건)의료 현황의 체계적인 모니터링 시스템에 필요한 데이터 조사체계 개편 및 조사지표 개발이 필요하다.

▢ Introduction
Ⅰ. Background
In the field of public health care, there are changes from a focus on possession and responsible organization to the interest of the public. However, there is a lack of in-depth discussion and research in the government and the field of dentistry on the concept and role of public oral health care (POHC), as well as essential oral health care (EOHC). It is necessary to establish the concept and role of POHC and to promote POHC policies.
In South Korea, the dental healthcare delivery system in South Korea is mainly provided by the private sector. As of 2022, the proportion of public healthcare institutions in the dental care system was 3.34% for dental hospitals and 0.07% for dental clinics. The reason for the low medical care insurance for primary care such as prevention, periodontal care, and conservative treatment, active management of oral diseases is not actively pursued in the private sector however prosthetics and non-reimbursement treatment account for a significant portion. The types of oral health care services provided by public health centers are limited and restricted to certain individuals, and there is a significant regional disparity in the availability of medical resources. Oral health care professionals by national agencies responsible for developing and implementing national oral health policies are either nonexistent or very limited compared to other health care professionals.
The 「Korea National Children's Oral Health Survey」 and the oral health sector in 「Korea National Health and Nutrition Examination Survey」 are representative oral health care survey monitoring systems for the Korean population. However, due to Cross-sectional study design, insufficient budgets, short survey periods, the absence of specialized survey teams, and the difficulty in recruiting dentists as surveyors, it is increasingly challenging to produce high-quality indicators of oral health. A review and improvement of the current oral health survey system are necessary for the development and evaluation of effective oral health policies.
Ⅱ. Objectives
This study aims to review the supply and operation status of public oral health care resources, the status of prevention and oral disease management, and the legal and administrative systems related to oral health care policies. The final goal is to establish and improve policies for POHC that can respond to needs of the future society by defining the concept and role of POHC. Therefore, this study conducted an exploratory research methodology to identify priorities for improving POHC. This involved a literature review, expert opinion surveys (including public hearings, focus group interviews, and online surveys), and case studies.
▢ Major Research Content and Result
Ⅰ. Trends in Public Health Care and Oral Health Care Policies
According to the 「Public Health and Medical Services Act」 extensively revised in 2012, public health care indicates activities undertaken by the nation, local autonomous entities, and healthcare institutions, regardless of regions, social standings, or fields. The purpose of these activities is to ensure universal healthcare coverage and encompasses all activities related to health promotion and protection. Currently, public health care continues to operate as a residual form of access, addressing vulnerable areas, vulnerable populations, and market failures. The 「Public Health Care Development Comprehensive Plan (2018)」 identified that the current functions of the public healthcare delivery system are inadequate to address the current issues and the increasing demand for medical services. In response to these challenges, the plan aimed to expand the role of public healthcare by proactively ensuring essential healthcare. And then The Public Health Care Development Committee discussed and restrict the scope of essential healthcare. The 「Regional Medical Strengthening Plan (2019)」 was announced to foster local medical resources and enhance regional medical cooperation, for mitigate local medical disparities in essential medical care. According to the 「2nd Public Health Care Basic Plan (2021)」, the definition of public health care underwent a shift from possession, responsibility organization, and supplementation to the realization of public interest for universal assurance.
Throughout the government's discussions on establishing a public health care system, there has been a lack of consideration regarding POHC. While departments of dentistry are a mandatory treatment subject in tertiary medical hospitals under the healthcare delivery system, its inclusion in departments of dentistry in secondary medical hospitals remains optional. Dentists are not required in emergency medical institutions. Despite dental hospitals being affiliated with 11 universities nationwide and playing a substantial role comparable to tertiary general hospitals in the oral healthcare delivery system, they have not been legally or institutionally distinguished from private dental hospitals and clinics. In contrast to the healthcare delivery system, which mandates patients to seek referrals for treatment at tertiary medical hospitals, university dental hospitals and departments of dentistry in tertiary general hospitals allow patients unrestricted access, indicating an absence of an established oral health care delivery system.
Ⅱ. Status of POHC
There are (specialist) dentists, public health dentists, dental hygienists, dental technicians, nurses, and nursing assistants who provided public oral health care services called the oral health care professionals. In South Korea, the number of active dentists per thousand people is relatively low at 0.5, according to OECD data from 2021. The majority of dentists work in the private sector, but only so much dentists in the public sector.
The 「Oral Health Act」 aims to efficiently promote oral health projects to prevent oral diseases and enhance people's oral health. According to this law, public health centers must establish and operate oral health rooms or centers for the prevention and treatment of oral diseases. Additionally, central facilities for dental treatment for people with disabilities must be established and operated to promote oral health. Regional facilities for oral health for people with disabilities could be established and operated. As of 2023, there is 1 central facility and 14 regional facilities are now operating. Among the 256 public health centers nationwide, 87% have oral health rooms, and 23% have oral health centers. However, they have differences as shortage of dental professionals, insufficient budgets, and inadequate leadership and capabilities.
According to the 「Seoul National University Dental Hospital Establishment Act」 and the 「National University Dental Hospital Establishment Act」, dental hospitals at national universities, including those at Seoul National University, have a duty to perform public health care services under Article 2, Paragraph 2 of the Law on 「Public Health and Medical Services Act」. These institutions mainly target vulnerable populations by offering reduced treatment fees and mobile medical services. However, some dental hospitals do not disclose information related to public health care plans.
Among the 38 regional hub public health care institutions (comprising regional medical centers and Red Cross hospitals), 28 provide dental treatment services. There are difference in human resources and facilities among these institutions.
To enhance the public nature of oral health care, coverage for dental treatments under health insurance has been expanded. For children, this includes expanded coverage for oral examinations, pit and fissure sealants, and light-cured composite resin fillings. Adults are now covered for oral examinations and scaling. Seniors benefit from coverage for dentures, dental implants, and oral management services in homes and facilities under the 「Long Term Care Insurance Act」. The establishment of oral health centers, a dentist program (pilot) for disabled individuals and non-covered dental treatment cost support for severely disabled individuals are going on.

▢ Conclusions
This study established the concept, roles, and scope of POHC and developed a definition for EOHC. Through gathering opinions from relevant experts, we have developed a direction and detailed aspects. To assess the current status of EOHC, we conducted mapping analyses for each sub-category, aiming to derive improvement strategies for POHC policies.
A significant portion of EOHC is covered under non-insured medical services, This study proposes to priority provide essential and universally accessible services to all citizens. To resolve oral health inequity issues among disabilities and vulnerable populations, we suggest a direction for oral health care policy that considers oral health equity.
Utilizing private medical institutions, which constitute a majority of the dental service system, we need to actively consider expanding preventive-centered EOHC services. In the future, research is essential to establish a collaborative system between public healthcare institutions and private medical institutions, aiming to enhance public health insurance.
Integrated into the medical delivery system, the dental field demonstrates an inconsistency, particularly in categorizing dental hospitals (affiliated with a university) as secondary medical institutions, despite their role in providing advanced dental care and training. The current 「National Dental Hospital Establishment Act」 mentions the role of public health care operations, but the operation in the public sector remains limited. Therefore, this study proposes the development of implementation specifically strategies for the obligation of POHC and the provision of EOHC.
In public health centers and regional hub public medical institutions, there is an insufficient proportion of oral health care professionals. Specifically, personnel responsible for oral health care projects at the metropolitan (or provincial) level are often non-specialists, handling multiple tasks concurrently. This situation poses challenges in the effective operation of oral health care projects. To strengthen POHC, continuous efforts should be made to provide stable operational support, such as securing personnel and resources.
In summary, to ensure universal access to dental care, it is necessary to establish a delivery system centered on primary medical institutions for EOHC services. Additionally, the development of operational manuals for delivering EOHC services.
