1. 연구배경
뇌졸중은 전세계적으로 사망의 주요 원인으로 우리나라의 경우 2009년 통계청 자료에 의하면 뇌졸중은 주요 사망원인 2위로 인구 백만명당 52명이 뇌졸중으로 인해 사망하는 것으로 나타났다.
뇌졸중으로 인해 긴 장애가 나타나게 되므로 재활치료가 필수적으로 따라온다. 뇌졸중 재활치료의 목표는 이차 합병증을 예방함과 동시에 생존자들의 독립적 기능 회복 뿐 아니라 삶의 질을 향상시키는데 있다.
현재 뇌졸중 재활치료와 관련한 여러 연구가 있지만 뇌졸중 재활치료와 관련한 의료이용의 현황에 대해 전 국민 규모 및 뇌졸중 재활치료를 받는 환자의 의료비용의 규모도 아직 알려진 바가 없다. 이에 뇌졸중 재활치료에 소요되는 사회적 비용과 규모에 대한 전체적인 연구 뿐만 아니라 실제 재활치료와 관련한 의료이용을 하고 있는 환자와 가족의 입장에서 어떤 요구가 있는지 살펴보는 것도 필요하다.
2. 연구방법
(1) 뇌졸중 환자 보호자 대상 면접조사
뇌졸중 의료이용자의 요구 분석을 위해 뇌졸중 환자의 보호자를 대상으로 표적집단면졉(Focus group interview)을 통해 연구하였다. 2차에 걸쳐 각각 6명의 보호자나 간병인을 대상으로 의료이용과 관련한 질문을 하였고 이에 대한 대답을 주제별 분석을 통해 환자나 그 가족들이 뇌졸중 재활치료와 관련하여 어떤 의료이용을 하였으며 어떤 요구사항이 있는지 분석하였다.
(2) 뇌졸중 재활치료의 의료이용 분석
국내에서 뇌졸중 환자의 연도별 치료현황 및 추적관찰을 위하여 건강보험심사평가원(이하 심평원)의 요양기관 청구자료 중 2007~2010년 4년간 청구자료를 이용하여 연도별 뇌졸중 환자의 특성 및 재활치료 현황을 분석하였다.
(3) 뇌졸중 재활환자의 의료비용 분석
심평원의 청구자료 분석과 국민건강보험공단(이하 공단) 자료, 장애인 실태조사, 국민건강영양조사, 의료패널자료 등을 이용하여 2010년의 뇌졸중 및 뇌졸중 재활치료 의료비용을 산출하였다. 비용항목으로는 공식적 의료비용과 비공식적 의료비용, 교통비용, 간병비용을 선정하였다. 심평원 청구자료의 분석을 통해 결정된 대상자 717,583명을 대상으로 뇌졸중의 원인에 따라 뇌경색과 뇌출혈로 구분하여 입원과 외래비용을 분석하였다.
3. 연구결과
(1) 뇌졸중 환자 보호자 대상 면접조사
뇌졸중 발병 시 이를 뇌졸중으로 잘 인식하지 못하여 적절한 대처가 이루어지지 못한 경우가 많았고 환자가 의식을 잃을 경우 응급실을 방문하더라도 신속한 조치가 이루어지지 않거나 의료진간 협진이 잘 안 되는 경우가 있었다.
재활치료에 있어서는, 종합병원의 경우 환자에게 필요한 재활치료를 여러 가지 받을 수 있으나 환자가 많고 정해진 시간을 지켜야 하는 불편함이 있었다. 병원 이외 주거지 근처 복지센터에서 재활 프로그램을 이용하는 경우가 많았고 이러한 프로그램에 대한 만족도가 매우 높지는 않았으나 비용이 저렴하고 어느 정도는 환자의 재활에 도움이 된다고 느끼고 있었다. 재활 전문병원은 체계적으로 한다는 인식을 가지고 있고 재활치료가 필요하다는 생각을 하고는 있으나, 재활 병원 수가 많지 않고 적절한 곳을 찾기가 어려웠다. 요양시설과 같은 다른 기관에 비해서는 비용이 비싼 만큼 만족하는 부분도 있었다. 하지만 보호자로서 재활에 들어가는 시간과 비용에 부담을 느끼고 있었다.
노인장기요양보험 등 사회적 지원체계에 대해서는 어느 정도 인식하고 있는 경우도 있었고 재활 병원 외에도 노인 복지관, 장애인 복지관 등 지역 사회 복지 시설을 이용하는 경우 전문성이 떨어지거나 교육 상태가 미흡하다는 인식이 있었다. 대부분 비용 측면에서 큰 불만족을 나타냈는데 급성기의 치료비에 비해 만성기에 들어가는 의료비용에 더욱 부담을 느껴 비용적으로 사회적 지원을 받고 싶어 하였다.
(2) 뇌졸중 재활치료 환자의 의료이용 분석
심평원 청구자료를 분석한 결과 뇌졸중 진단(I60-I64, I69)으로 심평원에 청구된 환자는 2007년 756,172명(남 357,354명, 여 398,818명)에서 2010년 884,248명(남 425,151명, 여 459,097명)이었는데 이를 100,000명당 조 청구율로 보면 2007년 1,967명에서 2010년 2,191명으로 증가하였으나 연령표준화 청구율은 십만명당 2007년 1,967명, 2008년 2,017명, 2010년 1,990명으로, 2007년 대비 증가하였으나 2008년부터는 감소추세를 보였다. 2007년~10년 전체적으로 볼 때 환자의 평균연령은 66세(±12.2)세이고 60대, 70대가 가장 많았다. 상급종합병원(24.0%)과 종합병원(38.2%)에서 진단받은 환자가 62.2%, 신경과에서 이용 환자가 41.0%로 가장 많았다. 뇌졸중 환자가 받는 재활치료는 기본물리치료와 전문재활치료가 가장 많았는데 기본물리치료는 연도에 따라 감소하는 경향을 보였으나 전문재활치료는 증가하는 경향을 보였다.
(3) 뇌졸중 재활환자의 의료비용 분석
심평원 청구자료의 분석결과를 바탕으로 뇌졸중 환자의 의료비용을 분석하였는데 주요 동반 중증질환의 경우 진료비가 높아 1인당 진료비가 더 높게 추정되어 이를 제외한 명세서를 대상으로 분석된 결과를 정리하였다(각각의 분석내용은 부록에 정리함). 2010년 뇌졸중으로 인한 전체 환자의 치료비용은 보건의료체계 관점으로 보았을 때 2조 4,141억원 규모로 추정되었으며 이는 2010년 GDP 대비 2.32%를 차지하였다. 입원비용이 72%를 차지하였고 교통비, 간병비 등과 같은 비의료직접비용은 상대적으로 적게 발생하였다. 이를 보험자관점, 환자관점 및 여러 가지 민감도 분석을 해본 결과 최대 3조 7,396억원에서 최소 1조 91억원으로 추정된다.
뇌졸중 환자에서 재활치료를 한번이라도 받은 환자를 대상으로 의료비용을 추계하였는데 2010년 재활종류에 무관하게 재활치료를 한번이라도 받은 사람들을 대상으로 추정된 비용은 1조 5,024억원 규모로 전체 뇌졸중에 비해 절반 수준이었고 2010년 GDP 대비 1.44%를 차지하였다. 이중 입원비용의 비중이 77% 수준으로 이는 뇌졸중으로 재활치료를 받지 않는 환자들의 입원비용은 65%인 것에 비하면 재활치료를 입원상태에서 많이 받고 있기 때문으로 추정할 수 있다. 관점의 차이와 시간비용 포함 여부 등 민감도 분석을 하였을 때 최대 2조 2,344억원에서 최소 1조 5,024억원으로 추정되었다. 이를 1인당 연평균 비용으로 보았을 때 재활 치료를 재활치료를 한번 이상 받은 환자의 경우 15,230,685원으로 재활치료를 한번도 받지 않은 환자의 1,412,512원보다 10배 이상 높은 결과를 보였다. 뇌경색으로 인해 재활치료를 받은 환자들의 총 비용은 7,904억원의 규모였으며 뇌출혈로 인해 재활치료를 받은 환자들의 총 비용은 4,464억원의 규모였다. 뇌경색에 비해 뇌출혈로 인해 재활치료를 받은 환자들의 총 비용의 규모는 작았으나 1인당 진료비를 살펴보면 뇌출혈은 1년간 1인당 평균진료비가 1,129만원인데 비해 뇌경색은 673만 원 정도로 상대적으로 뇌경색 환자가 더 적게 들었다.
4. 결론
본 연구는 뇌졸중 환자에서 심평원 청구자료 상 재활치료를 한번이라도 받은 환자의 의료비용을 추계한 연구로 뇌졸중 후 재활단계에 있는 환자들을 대상으로 한 첫 연구라고 할 수 있다. 분석 결과 뇌졸중 재활환자는 높은 의료비용을 보인다. 물론 본 연구가 순수한 재활치료비용을 추정한 것은 아니므로 해석에 주의를 기울일 필요가 있다.
본 연구에서는 뇌졸중 후 재활치료의 단계에 있는 환자의 의료이용 자료를 산출하고 사회적으로 소요되는 의료비용을 추계하였다. 그리고 의료이용자의 요구사항을 알아보았다. 이러한 연구 결과는 향후 뇌졸중 및 재활치료 환자와 관련한 보건정책 및 뇌졸중 재활치료 후속 연구 등에 중요한 근거 자료로 활용될 수 있을 것이다. 향후 뇌졸중 재활환자의 의료이용, 순수한 재활치료의 비용 등에 대한 연구를 진행할 것을 제안한다.
1. Study Background
Stroke is a major cause of death worldwide. In Korea, stroke is the second most common cause of death and 52 people per one million population die due to stroke, according to the 2009 data of Census Bureau.
Rehabilitation therapy is essential because there are long-term disabilities due to stroke. The goal of stroke rehabilitation is to prevent secondary complications, recover the independent functions of survivors, and improve quality of life.
Currently, there are several studies related to stroke rehabilitation. But the status of health care utilization associated with stroke rehabilitation, including the scale of the whole population and health care costs of patients receiving rehabilitation treatment for stroke, are not yet known. Hence, it is necessary to look into any demands from the perspective of the patient and his/her family who are actually utilizing health care
associated with rehabilitation as well as studying the social costs spent in stroke rehabilitation and its scale.
2. Study Method
(1) Interviews with families and caregivers of stroke patients
The study was done through focus group interviews of stroke patients’ families and caregivers to analyze the needs of health care utilizers for stroke. Through two stages, each of the six members of families or caregivers was questioned about health care utilization. Based on the analysis of the answers by each topic, types of health care used by patients and their families for stroke rehabilitation and types of demands they had were analyzed.
(2) Analysis of Health Care Utilization for Stroke Rehabilitation
The annual characteristics of stroke patients and status of rehabilitation were analyzed using four years of data from 2007 to 2010 among claims data of medical institutions of the Health Insurance Review Agency (HIRA) in order to monitor annual treatment status of stroke patients in Korea.
(3) Analysis of health care costs in patients receiving rehabilitation for stroke
Health care costs for stroke and stroke rehabilitation in 2010 were calculated using the analysis of requested data from HIRA, data of the National Health Insurance Corporation (NHIC), the Disabled Survey, Korea National Health & Nutrition Examination Survey (KNHANES), and Korea Health Panel survey (KHP). Formal and informal health care costs, transportation costs, and nursing costs were selected for cost items. A total of 717,583 subjects, which were selected through analysis of claims data of HIRA, were divided into cerebral infarction and cerebral hemorrhage depending on the cause of stroke, and costs of hospitalization and outpatient service were analyzed.
3. Study Result
(1) Interviews with families and caregivers of stroke patients
There were many cases that appropriate management was not done at the onset of stroke because it was not recognized as stroke. In some cases of unconscious patients, rapid management was not done or cooperation between medical teams was poor even if the patient visited the emergency room of hospital.
As for rehabilitation treatment, general hospital patients could receive many types of rehabilitation treatments necessary for the patients but there were many patients and there were inconveniences of having to be on time. Use of rehabilitation programs at local welfare centers near the residences other than the hospitals was common. These programs were not very satisfactory but the families felt that the costs were less expensive and that it helped with the rehabilitation of patients in some
degree. The interviewed had recognized that rehabilitation-specialized hospitals were systematic and that rehabilitation was necessary, but there were a small number of rehabilitation hospitals and it was difficult to find a suitable hospital. Their costs were expensive compared to other institutions such as nursing homes but there were parts satisfying enough for the costs. However, time and cost of rehabilitation were burdens to patients and their families.
There were cases that the social support system, such as long-term care insurance for the elderly, was recognized in some degree. In the case of using local social welfare facilities like the senior center and the welfare center for the disabled, the centers were recognized as being less specialized or being poorly educated. Mostly, dissatisfaction with the costs and health care costs for chronic phase was more a burden compared to the costs for acute phase so the interviewed wanted to
receive social support in terms of costs.
(2) Analysis of Health Care Utilization for Stroke Rehabilitation
According to the HIRA claims data analysis, the number of patients who were billed to HIRA with diagnosis of stroke (I60-I64, I69) was 756,172 (Male: 357,354, Female: 398,818) in 2007 and 884,248 (Male: 425,151, Female: 459,097) in 2010. When the data were looked at as crude claims rate per 100,000 people, it increased from 1,967 in 2007 to 2,191 in 2010. The age-standardized rate of claims increased to 1,967 in 2007, 2,017 in 2008, and 1,990 in 2010 per 100,000 relative to 2007.
But it showed a declining trend since 2008. The average age of patients from 2007 to 2010 was 66 years old (±12.2), and sixties and seventies were most common. Patients who were diagnosed in the Advanced General Hospital (24.0%) and General Hospital (38.2%) were 62.2% and the Neurology department had the most patients who used health care for rehabilitation at 41.0%. Rehabilitation treatments that stroke patients receive are mostly ‘basic physical therapy’ and ‘specialized rehabilitation therapy’. However, the number of ‘basic physical therapy’ showed falling tendency while ‘specialized rehabilitation therapy’ showed rising tendency annually.
(3) Analysis of health care costs in patients receiving rehabilitation for stroke
The health care costs in stroke patients were analyzed based on the result of the HIRA claims data analysis. Because costs of medical treatment per person were estimated higher in the cases with severe comorbidity due to its higher cost of medical treatment, medical statements, excluding costs for the comorbidities, were analyzed and the results were organized.
From the healthcare system's perspective, total treatment costs of all patients for stroke in 2010 were estimated to be KRW 2.4141 trillion, which accounted for 2.32% of GDP in 2010. Hospitalization costs accounted for 72% while the direct non-medical costs, such as transportation and nursing costs, were relatively less. Analyzing this in
the perspective of insurers and patients as well as the analysis of various sensitivities showed estimation to be at a maximum KRW 3.7396 trillion to a minimum KRW 1.91 trillion.
Health care costs in stroke patients who received rehabilitation therapy at least once were estimated. The estimated costs in 2010 regardless of the type of rehabilitation therapy received were KRW 1.5024 trillion, which was half of the total costs for stroke, and it accounted for 1.44% of the GDP. Of these, the hospitalization costs accounted for 77%, whereas the hospitalization cost of patients who are not receiving rehabilitation was 65%. It could be presumed that it might be because
patients were receiving more rehabilitation therapy while they were hospitalized. It was estimated to be at a maximum KRW 2.2344 trillion and at a minimum KRW 1.5024 trillion, according to the analysis of sensitivities such as difference in perspective and whether time costs were included or not. Average annual cost per patient who received rehabilitation therapy at least once was KRW 15,230,685, which was over 10 times higher than KRW 1,412,512 in patients who never received rehabilitation therapy. The total costs in patients who received rehabilitation for cerebral infarction was KRW 7.904 trillion in size, and the total costs in patients who received rehabilitation for cerebral hemorrhage was KRW 4.464 trillion in size. The scale of the total costs
in patients who received rehabilitation for cerebral hemorrhage was small relative to cerebral infarction. However, the average cost of medical treatment per person for one year was KRW 11,290,000 in cerebral hemorrhage while it was relatively smaller in cerebral infarction with KRW 6,730,000.
4. Conclusion
It can be said that this study is the first study on patients in rehabilitation stage after a stroke since it is a study that estimated health care costs in patients who received rehabilitation therapy at least once based on the HIRA claims data. The result of the analysis shows high health care costs in stroke patients receiving rehabilitation therapy. The result needs to be interpreted with caution since this study did not estimate the net costs of rehabilitation therapy.
In this study, the healthcare utilization data of stroke patients in the rehabilitation stage was generated and the costs of health care spent socially were estimated. The demands of healthcare users were studied as well.
The result of this study may be used as an important foundation data for healthcare policy associated with stroke patients and patients receiving rehabilitation therapy, and for a stroke rehabilitation follow-up study in the future. Studies on the healthcare utilization of stroke patients receiving rehabilitation therapy and net costs of rehabilitation therapy are suggested in the future.
