1. 연구목적
우리나라의 국민건강영양조사자료에 의하면 성인에서 BMI 25kg/m2 이상의 비만인구는 1998년 26.0%에서 2009년 31.3%로 증가 추세에 있으며, 비만수술을 고려할 수 있는 고도 비만환자도 전체인구를 기준으로 동일기간에 2.4%에서 4.7%로 증가하였다. 비만으로 인한 합병증 증가, 사망률 감소 등은 잘 알려진 사실이지만, 우리나라에서 비만에 대한 치료방법확립을 위한 노력과 치료에 대한 사회적 지원은 미약한 것이 사실이다. 전세계적으로도 비만과의 전쟁을 선포하는 등 비만을 억제하기 위해서 외국은 지속적으로 정책적인 지원이 있었으나 국내에서는 비만과 관련된 모든 진료행위가 급여 범위에서 제외되고 있다. 그러나 비만환자, 특히 고도 비만환자는 다양한 동반질환이 위중하고, 이로 인한 고통 속에 있으며, 고도비만을 개선시키면 동반질환이 개선되며, 비만환자가 사회적⦁경제적 수준이 낮다는 연구들이 제시되면서, 비만수술에 대한 급여 필요성은 지속적으로 제기되고 있다. 또한 국내 고도 비만환자에서 시행되는 수술에 관련된 임상결과는 매우 적으며, 비수술요법과 비교임상결과는 없었다. 따라서, 본 연구는 국내 고도 비만환자의 현황을 파악하고, 고도 비만환자의 치료방법 중 하나인 비만수술에 대한 유효성과 안전성, 삶의 질 및 보건의료체계관점에서의 경제성을 우리나라 자료로 평가하여 고도 비만환자의 치료방법 선택에 도움을 주고자 하였다. 또한, 고도 비만환자들이 가지는 사회적인 어려움 및 인식을 파악하고, 이에 대한 사회적 합의를 통해, 고도 비만환자들에 대한 사회적 지원 필요성을 인식하는 데 도움을 주고자 하였다.
2. 연구방법
1) 후향적 의무기록 조사를 통한 비만수술의 유효성과 안전성 평가
2008-2011년 2월까지 BMI 30 이상인 18세 이상의 성인으로서 8개 3차 병원에서 비만수술을 받은 261명과 수술이외의 비만치료를 받은 224명을 대상으로 후향적 의무기록 자료를 분석하였다. 유효성을 보기 위하여, 기저치로부터 체중감소율, 초과체중감량률(%EWL, percent excess weight loss), 초과체질량감량률(%EBMIL, percent excess body mass index loss), 절대적인 체중감소가 측정되었다. 당뇨, 고혈압, 이상지질혈증과 같은 동반질환으로 부터의 관해도 조사되었다.
후향적 의무기록을 조사한 대상자들 중에서 현 의료기관을 방문하고 동의한 환자를 대상으로 삶의 질 설문지를 조사하였다. 사용된 삶의 질 도구는 EQ-5D, Impact of Weight on Quality of Life - Lite (IWQOL-Lite), Obesity-related Psychosocial Problems scale (OP-scale), Moorehead-Ardelt Quality of Life Questionnaire II(M-AQoLQ II) (수술군에 한해 질문)였다. 대상자들에게 현재시점의 삶의 질을 평가하였으며, 치료 전의 시점과 비교하기 위해서 수술군 및 비수술군 각각은 수술요법을 받기 전과 비수술요법을 받기 전 시점을 회상하여 응답하도록 하였다.
2) 비만수술의 경제성 분석
고도비만 환자에서의 경제성분석은 두 가지 모델을 사용하여 각각 결과를 분석하였다.
첫 번째 모델에서는 후향적의무기록조사 자료를 사용하여 국내의 실제 임상현실을 반영하여 당뇨가 있는 BMI 30 이상인 환자에 대해 임상효과지표를 사용하여 비용-효과 분석을 수행하였다. 관점은 보건의료체계관점으로 하였고, 분석기간은 1년, 분석모델은 decision tree를 사용하였다. 위밴드술, 루와이위우회술, 위소매절제술이 포함된 수술치료와 비수술치료를 비교대안으로 하였다. 효과는 기저치로부터 체중감소율로서 확인하였고 비용은 9개 병원의 전문가 설문조사를 주로 적용하였다. 전이확률도 후향적 의무기록조사를 자료원으로 하였다.
두 번째 경제성 분석 모델은 BMI 30 이상-40 미만의 고도비만 환자에서 비수술치료와 비교한 수술치료의 비용-효용 분석이다. 분석관점은 보건의료체계관점으로 하였고, 분석기간은 평생, 분석주기는 1년으로 하여 half-cycle correction을 적용하였다. 코호트 시작연령은 30세이고, markov model을 사용하였다. 모형의 건강상태는 No comorbidity, Mild/Moderate comorbidity, Severe comorbidity, Death due to CVD, Death due to other cause의 5가지로 구성하였다. 효과는 국민건강영양조사 자료를 사용하여 효용가중치를 적용하였고, 비용은 9개 병원 전문가 설문조사와 건강보험통계연보를 자료원으로 하였다. 전이 확률은 문헌과 의무기록조사, 국민건강영양조사 자료를 사용하였다. 사망률은 통계청에서 CVD로 인한 사망과 다른 원인으로 인한 사망을 각각 적용하였다.
3) 비만의 영향력 (자살시도와 동반질환을 중심으로)
2007-09년에 실시된 국민전체를 대표할 수 있는 국민건강영양조사자료 중 동반질환과 자살시도에 대한 비만의 영향력을 보기위하여 조사되었다. 동반질환의 종류와 유무는 동반질환에 대해서 의사에게 진단 받은 적이 있는지에 대한 질문에 대한 답변으로 정하였다. 자살시도는 자살생각이 있다고 대답한 사람들에게 자살시도를 했는지에 대한 질문에 대한 답변 (예/아니오)을 통해서 확인하였다.
4) 비만에 대한 질적연구와 의사의 일반적인 인식과 태도
본 연구대상자는 비만지수 30kg/m2이상이면서 연령이 20세 이상 50세 미만인 여성을 대상으로 포커스그룹미팅과 일대일 면접을 통해서 비만 환자들이 인식하고 있는 비만의 원인, 사회적 적응의 어려움, 비만을 악화시키는 요인 등을 확인하였다. 또한 인식에 따라 비슷한 의견이나 생각을 가진 사람들끼리 묶을 수 있는 Q방법론을 통하여 일반인과 의료인을 비만에 관한 인식과 태도에 따라 유형을 구별하였다. 부가적으로 비만과 고도비만, 고도비만의 치료에 대한 1차 의료기관에 종사하는 의료인의 태도와 인식을 조사하기위하여, 서울-경기도, 전라도, 경상도, 충청도, 강원-제주도 소재(각 지역마다 20명)의 일차 의료기관에 근무하는 내과, 가정의학과 의사 100명을 대상으로 설문조사를 시행하였다.
3. 연구결과
1) 후향적 의무기록 조사를 통한 비만수술의 유효성과 안전성 평가
수술군의 평균 BMI는 39.0(±6.2)이고 비수술군의 BMI는 34.3(±3.8)으로서 수술군이 높았으며, 사용된 수술방법은 루와이위우회술 73건 (28.0%), 위 밴드술 72건(27.6%), 위소매절제술 116건(44.4%) 이었다. 치료 후 18 개월까지 수술군과 비수술군의 추적율은 31.9%(44/139명) 와 21.4%(37명/173명)였고, 18개월 시점에 체중측정치를 가지고 있는 환자는 수술군이 261명 중 44명(16.9%), 비수술군이 224명 중 37명(16.5%)이었다. BMI 30 이상의 고도비만 환자에서 수술적 치료는 비수술적 치료에 비하여 체중 감소에 효과적이었다. 18개월째까지 추적된 고도비만환자에서 비수술군이 6.7%(95%CI: 4.4-9.1%)와 수술군이 22.6% (95%CI:19.1-26.0%)의 체중 감소율을 보였다. 초과체중감량률(%EWL, percent excess weight loss), 초과체질량감량률(%EBMIL, percent excess body mass index loss), 절대적인 체중감소에서 모두 동일경향을 보였다. 체중감소 효과의 차이는 BMI에 따라 다르지 않았다. 외국의 대규모 코호트 연구의 대조군에 비하여 본 연구에서는 대조군에서도 치료 효과는 존재하였지만 성공적인 체중감소 효과가 있었다고 볼 수는 없었다. 당뇨, 고혈압, 고지혈증 등의 동반 질환 개선 정도도 수술적 치료 군에서 더 우수하였다. 복강경위밴드수술, 루와이위우회술, 위소매절제술 세 술식을 비교했을 때, 체중감소 효과는 위우회술이 가장 우수한 경향을 보였으나, 일부 구간에서만 통계적 의의가 있었다. 위밴드와 위소매절제술간의 체중감소 효과는 유사하게 나타났다. 합병증 발생률은 30일 이내 10.0%, 30일 이후 12.3% 나타났다. 루와이위우회술군(12%)와 위소매절제술군(7%)에서 30일 전후로 유사하게 나타났으나, 복강경위밴드군에서는 30일 이후 합병증이 의미있게 높게 나타났다 (12.5% vs. 22.2%). 전체 사망률은 1명(0.38%)이었고 루와이위우회술군에서 발생하였다. 수술 후 비만관련 합병증 치료/개선에 있어서 당뇨병, 고혈압, 고지혈증 관해가 루와이위우회술 후 가장 높게 나타났다. 모든 삶의 질 도구에서 수술군이 비수술군에 비하여 삶의 질이 통계학적으로 유의하게 개선되었다.
2) 비만수술의 경제성 분석
고도비만환자에서 수술치료와 비수술치료의 비용효과 분석결과, 비수술치료에 비하여 수술치료를 하는 경우 분석기간 1년 동안 체중감소율 1%를 증가시키기 위하여 추가적으로 소요되는 의료비용이 약50만원이었다. 평생의 분석기간 동안 1인당 기대의료비용은 비수술군이 16,392,886원, 수술군에서 17,914,487원으로 수술군에서 1,521,601원 더 소요되는 것으로 나타났다. QALY를 사용한 효과를 분석했을 경우 비수술군에서 15.43, 수술군에서 16.29로 수술군에서 0.86 QALY가 더 높았다. 따라서 고도비만 환자에서 수술치료는 비수술치료에 비해 의료비용은 높으면서 효과도 좋은 대안이었으며, ICER가 1,770,535원/QALY로 산출되었다.
3) 비만의 영향력 (자살시도와 동반질환을 중심으로)
2007-09년에 실시된 국민전체를 대표할 수 있는 국민건강영양조사자료 중 19세이상의 성인 17,310명의 분석결과, 정상군에 비해 고도비만군은 1개 이상의 동반질환이 있을 가능성은 1.76배(95% CI:1.42- 2.19) 높았고, 자가스트레스는 1.38배(95% CI:1.14-1.68) 높았다. 자살생각은 1.30배(95% CI:1.001- 1.69) 높았으며, 자살시도는 2.14배(95% CI:1.12- 4.09) 높았다. 연령별 층화 분석 결과 40세이상 군에서는 동반질환에 더 영향을 미치고 19세 이상 40세 미만군에서는 동반질환의 존재 및 자살이나 스트레스와 같은 정신적 건강 모두에 영향을 미쳤다.
4) 비만에 대한 질적연구와 의사의 일반적인 인식과 태도
고도 비만 환자 10명을 대상으로 포커스그룹미팅과 일대일 면접을 통한 질적연구를 통해서, 고도 비만 환자들이 인식하고 있는 비만의 원인, 사회적 적응의 어려움, 비만을 악화시키는 요인 등을 확인하였으며, 체중조절을 위한 본인, 가족, 전문가, 사회경제적 지지체계의 필요성을 도출할 수 있었다. 또한 인식에 따라 비슷한 의견이나 생각을 가진 사람들끼리 묶을 수 있는 Q방법론을 통하여, 비만하지 않은 사람들 중 비만한 사람들에 대하여 낙인하거나 방관하는 유형이 있음을 확인하였다. 의료인 중에도 고도비만과 비만치료에 대해서, 보존적 치료 옹호자와 적극적 치료 옹호자의 2분류로 나누어지며, 인식의 불일치가 존재하였다.
1차 의료기관에 종사하는 의사 100명(전국) 설문 결과, 우리나라 1차 의료기관에서는 비만관리가 잘 이루어지고 있지 않고, 고도비만환자에 대한 사회적 지원이 거의 없으며, 가장 필요한 사회적 지원으로 의료보험이 필요하다고 생각하고 있었다. 몸매성형수술인 지방흡입술등과 비만수술을 혼돈하여 생각하는 경우가 많아서, 이에 대한 정확한 정보 전달 교육이 필요하며, 비만 치료 지침(2009, 비만학회)에는 고도비만의 유일한 치료방법으로서 비만수술을 언급하고 있으나, 이를 치료방법으로 고려하는 경우는 매우 낮아서, 1차 의료기관에 종사하는 의사들의 인식과 진료지침사이의 차이를 확인 할 수 있었다.
4. 결론
결론적으로, 비만수술과 비수술비만치료를 받은 고도비만 환자의 후향적 의무기록자료 분석결과, 18개월 추적 관찰기간 동안 수술적 치료는 비수술적 치료에 비하여 체중 감소에 효과적이었다. 당뇨∙고혈압∙고지혈증 등의 동반 질환 개선 정도 및 삶의 질 향상도 비만수술군에서 더 우수하였다. 비수술요법과 비교하여 수술요법에 대한 경제성분석 결과 비용이 1,521,601원 더 소요되지만 QALY가 0.86 높아 점증적 비용효과비가 1,770,535원/QALY으로 비용효과적인 대안임을 확인할 수 있었다. 고도 비만 환자 및 일반인을 대상으로 한 질적연구등을 통해 비만에 대해서 낙인하는 사회적 분위기와 이러한 사회적분위기가 비만을 악화시키는 요인임을 알 수 있었고, 체중조절을 위해서는 본인뿐만 아니라 가족∙전문가의 도움과 사회경제적 지지체계의 필요성을 확인하였다. 1차 의료기관에 종사하는 전국 의사 100명을 대상으로 한 설문 결과, 비만 치료 지침(2009, 비만학회)에서 제시한 근거가 있는 유일한 고도비만 치료방법인 비만수술을 실제 치료방법으로 고려하는 비율이 낮아서, 의사들의 인식과 진료지침 사이의 차이를 확인할 수 있었다. 의료인과 환자 설문조사 결과, 고도비만 환자에게 가장 필요한 사회적 지원은 건강보험이었다.
1. Objectives
Based on the national representative survey data, Korean National Health and Nutrition Examination Surveys (KNHANES), the prevalence of obesity has been increasing in South Korea. The prevalence of obesity defined as having a BMI >=25 kg/m2 increased from 26.0percent in 1998 to 31.3percent in 2009. During the same period, the prevalence of severe obesity as having a BMI ≥ 30 kg/m2 increased markedly (from 2.4 percent to 4.7 percent). It is well known that obesity is more
common in people with lower socioeconomic status, but social support has been very limited. All health services for the treatment of obesity including bariatric surgery are not covered by national health insurance in South Korea. Therefore, the purpose of this study was to evaluate the cost-effectiveness as well as effectiveness and safety of bariatric surgery in severely obese Korean people. In addition, we assessed the overall social difficulties on people living with obesity. This study would be helpful to understand the need of social support for people with severe obesity.
2. Methods
1) Effectiveness and safety of bariatric surgery
We used the retrospective cohort design using medical chart review to investigate the effectiveness and safety of bariatric surgery compared with conventional therapy. We identified the 261 patients who underwent bariatric surgery and 224 patients who were treated by conventional therapy such as weight control medication and lifestyle modification therapy in tertiary medical center from Jan 2008 to Feb 2011. To investigate the weight loss, the percent weight change from baseline, excess weight loss, excess Body Mass Index (BMI) loss, and absolute weight loss over time were calculated. The recovery from co-morbidity such as diabetes, hypertension, and dyslipidemia and the complication rates for bariatric surgery were also investigated.
The quality of life was investigated for the patients who re-visited the medical centers from Jul 2011 to Oct 2011 among patients who included in the retrospective cohort study. The instruments for quality of life were EuroQol-5D (EQ-5D), Impact of Weight on Quality of Life-Lite Questionnaire (IWQOL-Lite), Obesity-related Problem scale (OP-scale), and Moorehead-Ardelt Quality of Life Questionnaire II (M-AQoLQII).
2) Cost-effectiveness study for bariatric surgery compared with conventional therapy
The cost-effectiveness analysis was conducted using two models: decision model and decision-markov model. Model 1 was the decisionmodel with a one year time horizon, and Model 2 was the decision-markov model over a lifetime horizon. Two models used different outcomes: effectiveness associated with weight loss in the
Model 1 and quality adjusted life years (QALY) in the Model 2. In the decision-markov model, the starting age of cohort was 30 years old, and the health status was consisted of five states such as no comorbidity, mild/moderate comorbidity, severe comorbidity, death due to CVD, and death due to other cause. The cost data was collected from the survey of medical doctor who had conducted these obesity treatments and Korean national health insurance statistics. The effectiveness data of
Model 1 was collected from retrospective chart review, and that of model 2 was collected from the data from literature, KNAHES, Korean death statistics as the input data of transition probability for five status and utility weight. Incremental cost-effectiveness ratio (ICER) of bariatric surgery compared to non-surgery interventions was calculated in severely obese people.
3) Impact of obesity on co-morbidities and suicide attempt
We applied cross-sectional design using 2007-09 KNHANES data was intended to see the impact of obesity for co-morbidities and suicide attempt. Co-morbidities were determined on the basis of self reports of having been diagnosed by a physician. Suicide attempts were investigated by asking those who acknowledged suicidal ideation whether they had ever attempted to commit suicide. Responses to the question were treated as dichotomous variables (“yes” or “no”).
4) Qualitative research for obesity and Study of the knowledge, attitude, and practices in general practice
The qualitative research was conducted in 10 people with severe obesity to assess the cause of obesity, the social difficulties on people living with obesity, and the risk factors exaggerating the obesity. For this qualitative research, focus group interview and face-to-face interview were used. In addition, the survey of 100 general practitioners was conducted to see the knowledge, attitude, and clinical practice for the obesity treatment.
3. Results
1) Effectiveness and safety of bariatric surgery
The bariatric surgery consisted of Roux-en Y gastric bypass (RYGB) of 28.0 %, Laparoscpic Adjustable Gasrtic Banding (LAGB) of 27.6%, and Sleeve Gastrectomy (SG) of 44.4%. The percent weight change from baseline at 18 month was significantly higher in bariatric surgery group (22.6%, 95%CI: 19.1-26.0%) than conventional therapy group (6.7%, 95%CI: 4.4-9.1%). This pattern was consistent with other outcomes for weight loss. The recovery from co-morbidity such as diabetes,
hypertension, and dyslipidemia was better in surgery group. The complication rate among bariatric surgery was reasonable; one case of death (0.38%) occurred in RYGB. The improvement in quality of life was significantly higher among bariatric surgery group compared with the conventional therapy group.
2) Cost-effectiveness study for bariatric surgery compared with conventional therapy
Both models showed that bariatric surgery was cost-effective. In particular, cost-effectiveness study over lifetime indicated that bariatric surgery had higher costs (Incremental costs: USD1,521) and higher QALY (Incremental QALY : 0.86) through the base case analysis. Incremental cost effectiveness ratio (ICER) was USD1,771. Through the sensitivity analysis under a variety of assumption, robustness of the study results was also demonstrated.
3) Impact of obesity on co-morbidities and suicide attempt
Among 17,310 Korean adults, severely obese people showed significantly higher Odds Ratio (OR) for suicide attempt (OR: 2.14, 95%CI: 1.12-4.09) and for having a disease (OR:1.76, 95% CI:1.42- 2.19), when compared with normal weight people.
4) Qualitative research for obesity and Study of the knowledge, attitude, and practices in general practice
Obese people thought that their own will to control weight, support from family and health care professionals, and social support such as national medical insurance were essential to control obesity. Through the survey of 100 general practitioners, they showed a reasonable level of interest in participating in obesity prevention and management, but they rarely conduct it for even severely obese people in clinical practice. They thought that the social support was very limited for severely obese
people and the national medical insurance was essential method as the social support. But half of them could not distinguish the bariatric surgery from liposuction which has been used for cosmetic surgery to reform the body shape and most of them had lack of knowledge about the effectiveness and safety of bariatric surgery. Korean obesity guideline recommends bariatric surgery as the unique treatment for severe obesity. The appropriate training for the obesity management is needed for better practice.
4. Discussion and conclusion
In conclusion, bariatric surgery had significant weight loss and improvement of quality of life, when compared with conventional therapy in patients with severe obesity. The recovery from co-morbidity such as diabetes, hypertension, and dyslipidemia was better in surgery group. The complication rate of bariatric surgery was reasonable; one case of death (0.38%) occurred in RYGB. The cost-effectiveness study over lifetime indicated that bariatric surgery was cost-effective alternative to
conventional therapy, providing substantial lifetime benefits in severely obese people. In addition, both severely obese people and health care professionals thought that social support for severe obesity was very limited. Considering gaps between obesity management guideline and knowledge levels among general practitioners, the appropriate training for the obesity management is needed for better practice among general practitioners.
