▢ 연구 배경
「한국보건의료연구원 의료기술평가 국민참여단」은 의료기술평가 과정에 국민 목소리를 반영하여 환자중심의료를 실현하기 위한 목적으로 구성된 국민 자문그룹이다. 본 연구는 한국보건의료연구원 2019년 연구주제 수요조사에 국민참여단이 제안한 연구주제 중 채택 및 기획되어 수행되었다.
국민참여단에서 당초 제안한 연구제목은 “인공관절 수술 후 재활치료의 효과”이었다. 인공관절 수술 후 재활치료병원에서 비급여로 재활을 받을 경우 비용부담이 너무 큰 상황인데, 그럼에도 자가재활보다 비용효과성이 높은지 그리고 필수적으로 받아야 하는지에 대한 근거가 필요하다는 내용이었다.
이후 관련 임상전문가들과의 논의를 통해 연구대상과 연구범위를 재설정하였다. 연구대상은 슬관절 전치환술 후 재활치료를 받은 환자로 축소하였고, 연구내용은 환자 대상 설문조사를 통한 환자 경험 평가에 초점을 맞추는 것으로 조정하였다.
슬관절 전치환술 후의 재활과 관련하여 최적의 환자 의료결과에 이르게 하는 중재에 대한 합의가 없기 때문에 병원 및 의사마다 프로토콜이 다양할 수 있어, 환자와 의사 모두에게 도전과제이다(Mistry 등, 2016; Westby 등, 2010).
▢ 연구 목적
본 연구에서는 슬관절 전치환술 후 재활치료에 대한 국내 의료 현황과 실제 임상현장에서의 장애요인을 분석하여, 이에 대한 대응방안 마련 시 활용할 수 있는 객관적 근거를 제공하고자 하였다. 또한, 슬관절 전치환술 후 재활치료와 관련된 미충족 의료에 대한 개선을 위하여 일반인에게 정보를 제공하기 위한 정보집을 제작하고자 하였다.
▢ 연구 방법
1. 환자 대상 설문조사
슬관절 전치환술 후 수행되는 재활치료에 대한 현황과 만족도 및 장애요인을 파악하기 위하여 최근 6개월 이내에 슬관절 전치환술을 받은 경험이 있는 환자 100명을 대상으로 설문조사를 실시하였다. 표본추출은 목적적 할당 표본추출 방법을 활용하였다. 설문조사는 헬스케어 전문면접원이 구조화된 종이 설문지를 이용하여 2019년 6월 28일부터 7월 25일까지 일대일 개별 면접조사로 진행하였다.
설문조사 내용은 연구 대상자의 인구사회학적 특성(연령, 성별, 거주지 등), 슬관절 전치환술에 관한 특성(수술을 받은 원인, 수술을 받은 병원 유형 등), 슬관절 전치환술 후 재활치료의 특성(재활치료 유형, 병원 입원치료 여부, 재활치료에 대한 만족도 등)을 포함하였다.
2. 체계적 문헌고찰
슬관절 전치환술 후에 수행되는 비지도식 재활치료 대비 지도식 재활치료의 안전성과 유효성을 규명하기 위한 체계적 문헌고찰을 수행하였다. 문헌검색은 국외 4개 데이터베이스(Ovid-MEDLINE, Ovid-EMBASE, Cochrane Central Register of Controlled Trials (CENTRAL), Physiotherapy Evidence Database (PEDro))와 국내 2개 데이터베이스(KoreaMed, KMbase) 및 수기 검색을 병행하였으며, 출판연도 및 언어에 제한을 두지 않았다. 연구 설계가 무작위배정 비교임상연구인 경우만을 선택하였으며, 문헌 선택은 2명의 연구자가 독립적으로 검토한 뒤 의견 일치를 통하여 최종 문헌을 선택하였다. 최종 선택문헌에 대한 비뚤림 위험 평가는 Cochrane의 Risk of Bias 도구를 사용하였다. 자료추출 후 가능할 경우 임상 결과변수에 대한 메타분석을 수행하였고, 양적합성을 수행할 수 없는 경우 질적으로 기술하였다.
3. 대국민 정보집 제작
슬관절 전치환술 후에 수행되는 재활치료와 관련하여 일반인들에게 필요한 정보를 제공하여 관련 의사결정 시 환자 및 환자 보호자가 참고할 수 있도록 하기 위하여 대국민 정보집을 제작하였다.
정보집의 내용은 무릎관절 전치환술 및 재활치료의 개요, 무릎관절 전치환술 후 수행되는 재활치료의 종류, 무릎관절 전치환술 전·후 시행되는 기능검사, 무릎관절 전치환술 후 병원에서 수행되는 재활치료의 건강보험 현황, 자주 묻는 질문의 순으로 구성하였다. 특히 ‘자주 묻는 질문’에는 대상이 되는 환자나 환자 보호자의 질문 빈도가 높았던 사항이나 주의가 필요한 사항을 엄선하여 질문화한 후 그에 대한 답변을 포함하였다.
본 정보집의 표적 독자는 고령의 일반인임을 고려하여, 간결하고 쉬운 고객맞춤형 자료를 작성하고자 하였다. 또한, 효과적인 내용 전달을 위하여 일러스트를 활용하였으며, 일반인의 이해를 돕고자 다소 어려운 의학용어에는 별도의 설명을 추가하였다.
▢ 연구 결과
1. 환자 대상 설문조사
연구 대상자는 대부분 연령 60세 이상(95.0%)의 여성(88.0%)이었고, 거주지는 서울(59.0%)이나 인천/경기(28.0%) 지역이 많았다. 슬관절 전치환술을 받은 원인(병명)은 대부분 퇴행성관절염(95.0%)이었고, 슬관절 전치환술을 받은 이유는 통증 감소 목적(98.0%)인 경우가 대부분이었다. 슬관절 전치환술을 받기 전의 환자 상태는 관절운동범위가 감소했고(76.0%), 보조기구의 사용 없이 자력으로 걸을 수 있었으며(69.0%), 보행 시 무릎 통증이 심했던 경우(85.0%)가 많았다.
슬관절 전치환술 후 재활치료 유형은 병원재활 단독(69.0%), 병원재활과 자가재활 병행(28.0%), 자가재활 단독(3.0%)의 순으로 나타났다. 병원재활을 받은 환자(97명)는 입원치료를 받은 경우가 대부분이었다(97.9%).
슬관절 전치환술을 받은 병원 유형은 재활 병원(77.0%)이 가장 많았다. 입원치료를 통해 병원재활을 받은 환자(95명) 중 슬관절 전치환술을 받은 병원과 입원한 병원이 상이한 경우도 상당수(46.3%) 있는 것으로 나타났다. 수술한 병원과 다른 병원에 입원을 했던 이유로는 재활 병원에서 치료받기 위한 목적인 경우가 과반이었다(54.5%).
슬관절 전치환술 후 재활치료 방법은 관절가동범위 향상 훈련(99.0%)을 받은 경우가 가장 많았다. 그 다음으로는 근력강화훈련, 보행훈련, 균형훈련, 일상생활 수행능력 향상훈련의 순으로 나타났다. 수행한 재활치료 방법별 만족도는 큰 차이가 없는 것으로 나타났다.
슬관절 전치환술 후 재활치료에 대한 만족도는 대체적으로 만족스러웠던 것으로 나타났다. 만족하다고 응답한 비율은 재활치료 결과(83.0%), 수술부위의 기능적 측면(87.0%), 심리적 측면(82.0%)에서 모두 높았다. 또한, 슬관절 전치환술 후 병원 재활치료에 대한 만족도는 인력(치료사)(84.5%)이나 환경(기구 및 시설)(81.4%)에 대해서는 만족했다고 응답한 비율이 비교적 높았던 반면, 비용(34.0%)에 대해서는 만족했다고 응답한 비율이 상대적으로 낮았다. 이를 통해 우리나라에서 수행되는 슬관절 전치환술 후 병원 재활치료 서비스 자체에 대한 환자 만족도는 높은 편이나, 경제적인 측면에 대해서는 다소 불만족하고 있는 것으로 나타났다.
슬관절 전치환술 후 자가재활을 수행한 환자(31명; 병원재활과 병행한 28명 포함)에게 조사 시 혼자 할 수 있을 것 같아서 선택했다는 응답이 가장 많았으나(64.5%), 실제 자가재활을 수행할 경우 어려웠다고 응답한 경우는 41.9%이었다. 자가재활이 어려웠던 이유는 응답비율이 높았던 순으로 재활운동을 도와줄 사람이 없어서, 꾸준히 하게 되지 않아서, 재활운동 방법을 몰라서의 순이었다.
슬관절 전치환술 후 재활치료의 필요성에 대한 질문에는 슬관절 전치환술 후 재활치료가 필요하다고 응답한 경우가 대다수(99.0%)이었고, 그 중 개인재활보다는 병원재활을 선택하겠다(88.9%)고 응답한 비율이 높았다. 해당 응답자 중 통원치료보다는 입원치료를 선택하겠다고 한 비율이 높았고(89.8%), 그 중 절반 가량은(52.3%) 수술한 병원과 동일한 병원에서 재활치료를 받아야 한다는 의견을 제시하였다. 이를 통해 슬관절 전치환술을 받은 환자는 재활치료에 대한 높은 필요성을 느끼고 있고, 그 중에서도 입원방식을 통한 병원재활치료를 받을 의향이 높음을 확인할 수 있었다.
2. 체계적 문헌고찰
슬관절 전치환술 후 수행되는 지도식 재활치료와 비지도식 재활치료를 비교한 총 11편의 무작위배정 비교임상연구가 선택되었다. 주요 결과지표는 유효성과 관련한 환자보고 기능결과, 환자보고 통증/불편감, 삶의 질, 환자 만족도, 평가자측정 기능결과, 환자보고 강직도였다.
전반적인 결과를 요약하면, 거의 모든 의료결과에서 지도식 재활치료와 비지도식 재활치료 간에는 유의한 차이가 없는 것으로 나타났다. 단, 환자 만족도와 평가자가 측정한 결과지표(근력, 보행 및 균형능력) 일부에서는 지도식 재활치료의 결과가 우수한 것으로 보고하였다.
3. 대국민 정보집 제작
본 연구에서는 “환자와 보호자를 위한 무릎관절 전치환술 후 재활치료 안내서”를 제작하였다.
“1장. 무릎관절 전치환술 및 재활치료의 개요”에는 무릎관절 전치환술의 정의 및 질병부담, 무릎관절 전치환술의 대상자 및 금기증, 무릎관절 전치환술 후 재활치료의 필요성 및 기대효과에 대한 내용을 포함하였다.
“2장. 무릎관절 전치환술 후 수행되는 재활치료의 종류”에는 병원에서 수행할 수 있는 재활과 가정에서 수행할 수 있는 재활로 구분하여 소개하였다. 특히, 가정에서 수행할 수 있는 재활치료는 수술 후 시기별로(수술 직후, 수술 후 병원 내에서 걸음을 걸을 수 있게 되었을 때, 독립적으로 걸음을 걸을 수 있게 되었을 때) 적절한 재활에 대하여 설명하였다.
“3장. 무릎관절 전치환술 전·후 시행되는 기능검사”에는 무릎관절 전치환술 전·후에 시행될 수 있는 관절가동범위검사, 도수근력검사, 일상생활동작검사, 보행능력평가, 등속성운동기능검사, 버그 균형검사에 대하여 설명하였다.
“4장. 무릎관절 전치환술 후 병원에서 수행되는 재활치료의 건강보험 현황”에는 가장 보편적인 재활치료로서 건강보험으로 적용되는(급여) 복합운동치료와 미적용되는(비급여) 도수치료에 대한 정보를 수록하였다.
“5장. 자주 묻는 질문”에는 대상이 되는 환자나 환자 보호자의 질문 빈도가 높았던 사항이나 주의가 필요한 사항을 엄선하여 포함하였다. (ⅰ) 무릎관절 전치환술 전에 주사나 침 치료를 받아도 되는지 여부, (ⅱ) 무릎관절 전치환술을 받은 후 재활치료를 꼭 받아야 하는지 여부, (ⅲ) 무릎관절 전치환술을 받은 후 재활치료를 어디에서 얼마동안 받아야 하는지 여부, (ⅳ) 무릎관절 전치환술을 받고 퇴원 후 집에 있을 때 반드시 운동을 해야 하는지 여부, (ⅴ) 무릎관절 전치환술 후 가장 아픈 시기는 언제이며, 통증은 언제까지 지속되는지 여부에 대한 내용을 포함하였다.
▢ 결론
본 연구는 환자중심의료를 실현하기 위한 목적으로 구성된 국민 자문그룹인 「한국보건의료연구원 의료기술평가 국민참여단」의 주제 제안을 통해 기획되어 수행되었다.
슬관절 전치환술 후 재활치료를 받은 경험이 있는 환자를 대상으로 설문조사를 수행한 결과, 우리나라 환자들의 경우 자가재활보다는 병원(특히, 재활 병원)에 입원하여 의료진의 중재를 통한 지도식 재활치료를 많이 받는 경향이 있음을 확인하였다. 또한, 병원 재활치료의 경우에는 서비스 자체(기구, 시설, 인력 등)와 치료 결과에 대한 환자 만족도는 높은 편이나, 경제적인 측면(비용)에 대해서는 다소 불만족하고 있는 것으로 나타났다. 반면, 자가재활이 어려운 이유는 재활운동을 도와줄 사람이 없고, 꾸준히 하기가 어려우며, 재활운동 방법을 모르기 때문이라는 응답이 많았다.
체계적 문헌고찰 결과에서는 지도식 재활치료와 비지도식 재활치료 간의 의료결과에 뚜렷한 차이가 없는 것으로 나타났다. 그러나, 모두 국외에서 수행된 연구를 통해 도출된 결론이므로, 각 국가의 보건의료체계가 다름을 고려시 적용 가능성 측면에서 제한점이 있을 것으로 사료된다.
추가로, 본 연구를 통해 개발한 “환자와 보호자를 위한 무릎관절 전치환술 후 재활치료 안내서”가 고령의 일반인들에게 슬관절 전치환술 이후의 적절한 재활 및 관리방법과 관련하여 필요한 정보를 제공하여 정보의 부족으로 발생한 미충족 의료를 개선하는데 도움을 줄 수 있을 것으로 기대한다.
▢ Background
The Healthcare Technology Assessment Public Participation Group of the National Evidence-based Healthcare Collaborating Agency (NECA) is a public advisory group composed of members of various patient citizens’ organizations. Its aim is to implement patient-centered healthcare by reflecting the voice of the citizens in the healthcare technology assessment process. This study was carried out after being chosen from the research topics proposed by this group in the Agency’s Research Topics Demand Survey.
The study title originally proposed by the group was “The Effect of Rehabilitation Therapy after Artificial Joint Surgery.” When patients receive rehabilitation at a hospital after artificial joint surgery, they face heavy costs because the treatment is not covered by national health insurance. The study therefore aimed to determine whether such treatment was necessary and whether it was more cost-effective than self-rehabilitation at home.
The subjects and scope of the study were established through discussion with the relevant clinical experts. The study’s scope was limited to rehabilitation therapy after total knee arthroplasty (TKA), with the study content focused on evaluating patient experience, using patient surveys, and a comparative analysis, conducted through a literature review, of supervised and unsupervised rehabilitation.
▢ Objective
This study aims to analyze the healthcare status of post-total knee atrhtoplasty (TKA) rehabilitation treatments in South Korea, identify the obstacles to rehabilitation, and provide objectively based data that can be utilized to prepare appropriate countermeasures to such obstacles.
▢ Methods
1. Patient Survey
In order to evaluate the current status of TKA rehabilitation, patient satisfaction, and obstacles faced in TKA rehabilitation, a survey was conducted on 100 patients who had received TKA within the preceding 6 months due to degenerative knee osteoarthritis. The survey was conducted between June 28 and July 25, 2019, in the form of one-on-one interviews with a professional healthcare interviewer using a structured paper questionnaire.
The survey questionnaire items included demographic characteristics of the subjects (age, sex, place of residence, etc.), the characteristics of their TKA (cause of surgery, type of hospital where the patient underwent the surgery, etc.), and the characteristics of their rehabilitation after TKA (type of rehabilitation treatment, whether the patient was hospitalized, satisfaction with rehabilitation, etc.).
2. Systematic Literature Review
A systematic literature review was conducted to investigate the safety and effectiveness of supervised rehabilitation in comparison with unsupervised rehabilitation after TKA. The literature search was conducted using four international databases (Ovid-MEDLINE, Ovid-EMBASE, Cochrane Central Register of Controlled Trials [CENTRAL], and the Physiotherapy Evidence Database [PEDro]), two domestic databases (KoreaMed and KMbase), and manual searches. There was no restriction on year of publication or language used for publication, but only randomized comparative clinical studies were selected. Two researchers independently reviewed the literature, with the final decision on selection made through consensus after discussion. Assessment for risk of bias for the final selection was performed using Cochrane's Risk of Bias tool. After data extraction, a meta-analysis of clinical outcome variables was performed where possible, and the results were qualitatively described if quantitative fit could not be performed.
3. Production of Public Information Guides
In order to provide the public with the information necessary for rehabilitation after TKA and to make such information available to the patients and their caregivers for the relevant decision making, a public information guides booklet was produced.
The contents of the booklet were planned through discussion with the advisory group. The information in the booklet is organized in the following order: overview of TKA and rehabilitation treatment, types of rehabilitation treatment performed after TKA, function tests performed before and after TKA, health insurance status of rehabilitation treatment performed at hospitals after TKA, and frequently asked questions. The frequently asked questions portion includes selected questions that are frequently asked by patients or their caregivers, as well as matters requiring attention. The relevant answers are also included.
Since the target readers of this booklet are members of the general public, the contents are designed to be concise and easy to understand. In addition, illustrations are used for effective content delivery, and individual explanations are provided for the more difficult medical terms to help the general public understand them.
▢ Results
1. Patient Survey
Most of the subjects were women (88.0%), aged 60 years or older (95.0%), and most of them lived in Seoul (59.0%) or the Incheon/Gyeonggi (28.0%) area. The types of rehabilitation treatment were as follows: 69.0% received hospital rehabilitation alone, 28.0% received a combination of hospital rehabilitation and self-rehabilitation, and 3.0% underwent self-rehabilitation only. Of the patients who received hospital rehabilitation, 97.9% were hospitalized, with an average hospital stay of 2.6 weeks. Of the hospitalized patients, 46.3% underwent surgery in a hospital different from that where they were hospitalized. The main reason for changing hospitals was to receive their rehabilitation treatment in a rehabilitation hospital. In the case of self-rehabilitation, 41.9% answered that it was difficult to perform rehabilitation on their own, the most common cause of the difficulty being that they did not have anyone to help. The rate of satisfaction with hospital rehabilitation was high with regard to the personnel (84.5%) and the environment (81.4%), but relatively low in terms of the associated costs (34.0%). The satisfaction rate with rehabilitation was high in terms of the results of rehabilitation (83.0%), its functional aspects (87.0%), and its psychological aspects (82.9%). In total, 99.0% of the respondents said that they needed rehabilitation after TKA; 88.9% chose hospital rehabilitation, and 89.8% chose treatment with hospitalization.
2. Systematic Literature Review
A total of 11 randomized comparative clinical studies made up the literature. Effectiveness indicators reviewed were as follows: patient-reported functional outcomes, patient-reported pain/discomfort, quality of life, patient satisfaction, evaluator-assessed functional outcomes, and patient-reported stiffness. For the safety assessment indicators, complications or adverse events related to rehabilitation were analyzed. Almost all outcomes showed that there was no significant difference between supervised and unsupervised rehabilitation.
3. Production of Public Information Guides
Information guides were produced in order to provide information for use by patients and their caregivers when making decisions regarding rehabilitation after TKA. A booklet titled “Post-TKA Rehabilitation Guides for Patients and Caregivers” was produced.
In Chapter 1, “Overview of TKA and Rehabilitation Treatment,” the contents include the definition of TKA and the disease burden, the subjects and contraindications of TKA, and the need for and expected effects of post-TKA rehabilitation.
Chapter 2, “Types of Rehabilitation Performed after TKA,” introduces rehabilitation that can be performed in a hospital and rehabilitation that can be performed at home. In particular, the rehabilitation treatments that can be performed at home are described in terms of their suitability according to postoperative period (immediately after the surgery, when the patient can walk in the hospital after the surgery, when the patient can walk independently after the surgery).
Chapter 3, “Functional Tests Performed before and after TKA,” explains the ROM test, the manual muscle strength test, the activities of daily living test, the walking ability evaluation, the isokinetic motor function test, and the Berg balance test, which can be performed before and after TKA.
Chapter 4, “Health Insurance Status of Rehabilitation Treatments Performed in Hospitals after TKA,” contains information on exercise therapy and manual therapy, the most common rehabilitation treatments that are covered (paid) and not covered (non-paid), respectively, by national health insurance.
Chapter 5, “Frequently Asked Questions,” includes selected questions that are frequently asked by patients or their caregivers, as well as matters requiring attention. Examples include (i) whether injections or acupuncture may be given prior to TKA, (ii) whether rehabilitation must be received after undergoing TKA, (iii) where and how long rehabilitation should be received after undergoing TKA, (iv) whether exercise is needed after discharge from hospital after undergoing TKA, and (v) the most painful period after TKA and how long the pain lasts.
▢ Conclusions
As a result of conducting a survey of patients who have experienced rehabilitation after TKA, it was confirmed that Korean patients tend to receive supervised rehabilitation through the intervention of medical staff in hospitals rather than self-rehabilitation at home. Patient satisfaction with the hospital rehabilitation service was high, but satisfaction in terms of financial cost was rather low. The results of the systematic literature review showed no significant differences in outcomes between supervised and unsupervised rehabilitation. However, since this conclusion is drawn from studies that were all conducted abroad, there are limitations in terms of their applicability to Korea, given the different health systems in each country. It is expected that the booklet developed for this study, “Post-TKA Rehabilitation Guides for Patients and Caregivers” will help the general public obtain the necessary information regarding the appropriate rehabilitation and care after TKA.
